Backstory: I’m a male. I was infected with genital “HSV” in the mid 90’s. My memory is hazy but I definitely remember a gigantic cold sore on the outside of my GF’s mouth at one point, and we were very active (new relationship – probably had sex 3 times a week for 3 months). We continued to have sex. She never had any other symptoms. Was a dumb kid – next GF told me the cold sore was probably it and that’s pretty much how I’ve always assumed I had it. Never thought anything between differentiating, b/c no doctor ever told me I should.
To the point: After doing some research, obviously I want to know if I have gHSV1 or HSV2. I go on and off of antivirals depending on a few things. Recently single and assuming something may happen unexpectedly, I’ve been on antivirals for about 6 weeks (Valacyclovir 500mg x 1 per day). I was taking them prior to that but rather half-assing it. Forgot multiple days in a row, many times. Probably went 2 weeks without taking one in one stretch. Will this affect an IgG test and/or the WB? I took an IgG but waiting for delivery of the WB. I’ve seen you mention you need to stay off of antivirals for 12 weeks for WB and just wanted to confirm. Also curious by how much would the levels affect the IgG test? The labs make no mention of any of that, which seems quite irresponsible.

Most people seeking advice here are generally new. I’m curious what I should expect from IgG with confirmed 20 years exposure to one of the two?
(FWIW I just confirmed the ridiculous lab I went to get the IgG test only gives + or -. Useless).

Got my IgG results back (LabCorp).
I know this pushes my question back up, but it’s pertinent.
HSV 1 IgG, Type Spec <0.91 0.00-0.90 / index 01 (No value given, just <0.91)
FLAG RESULT Range/Units
HSV 2 IgG, Type Spec Above High Normal 1.75 0.00-0.90 / index 01
HSV-2 IgG Supplemental Test Abnormal Positive Negative
I know it’s looking rather slim that I have HSV-1, but from scouring other posts, it doesn’t look like the blot saying I have HSV-1 and not 2 wouldn’t be the craziest thing in the world. (Also curious about that “abnormal” flag on the supplemental). My symptoms have always lined up w/ 1 though. I do have recurrences, but probably 1.2 per year, and they are very mild (1, maybe 2 spots) even in the years that I thought I was a rock star and would abuse myself with somewhat hard drugs on most weekends. Daily antivirals take this to zero. 3 long term relationships sans condoms (and most of that time w/o suppression) and no known transmission. I realize it’s a fuzzy science though.
I’m curious how badly my antiviral regimen would screw up the IgG test. I get no numbers searching online. Honestly I would have loved to have seen the HSV1 come back positive but I do realize there’s a 30% shot that it’s straight up wrong.
Anyway, I’ve ordered the blot and set up a time w/ you next week. I guess I’ll know soon enough.

So if you’ve had this since the ’90s, antiviral medicine isn’t going to impact the results at all.
You definitely need the blot with that index value for HSV 2.
How exactly were you diagnosed originally?

I went to a few clinics. It is somewhat hazy but in my head:
– Initial outbreak was obvious. Diagnosis was visual. “You’ve got herpes”. I am pretty sure he just prescribed me valacyclovir (and 2 other medicines — one of which really messed with my head). It was pretty shocking, even to my partner (despite her cold sores — she look like she saw a ghost)
– Went to on campus clinic at least twice. I recall one doctor saying I had tested positive for HSV (maybe he said 2?) and another saying I tested negative at one point, but again I don’t remember which it was for. If it was these same tests, I mean, does it even matter? I assume they were even less accurate then.
So antivirals won’t affect the WB or the IgG considering I’ve had it 27 years? I guess I’ll move forward then.
I have an appt with you on Tuesday. We can discuss then. I have the WB kit en route.
