      I was not sure if I ran out of questions or not so please let me know if I need to buy more questions and I will!

      I have written to you in the past about my hsv2 anxiety which was triggered by having kids. I go to therapy but have had a recent setback.

      I am terrified of getting whitlow and I have bad eczema on my hands right now which has me upset. The only way I could get whitlow is if a break in my skin touched a herpes sore? I worry about indirect transmission like when I am in the shower; I feel the need to wear a glove to touch my wash cloth, and drying myself off after the shower. Incase the towel touches broken skin. Even today after using a glove while washing myself with a washcloth, I saw water from the shower got inside my glove and I became paranoid it was somehow water with herpes from my washcloth that got in my glove. Can herpes survive/be spread in water?

      Worrying about getting disseminated herpes or herpes myelitis is completely unnecessary?

      I have very sensitive skin and apparently sensitive genital skin. I know you have said sometimes herpes can present itself as just redness without any lesions. When my skin gets irritated my entire inner labia on both skins gets very red and inflamed. Even the opening of my vagina looks red and very swollen. Is this how you have seen herpes presented before? Or is it more of a small area that looks red with herpes. It is an extensive area for me and I am on suppressive therapy.

      There is no scenario you could think of that put my kids at risk of getting hsv2 anywhere on their body? You would never discourage someone with hsv2 from having children or another child?

      I hate how much emotional distress herpes gives me. I wish I could just accept my dx (I got it in 2009) and live a happy life but it seems like subconsciously I am convinced it will somehow ruin my life.

      Once you have HSV 2 in one location it is highly unlikely that you would get it in a new location.
      In regards to the shower: soap destroys the lipid layer that surrounds the herpes virus, so soap is definitely your friend in the shower. I would NOT suggest that you wear gloves in the shower – that is going way too far to deal with your concerns and you should try hard to make yourself stop doing that. Remember that soap is your friend!

      Herpes would not present as generalized red skin of the vulva, no, not at all.

      There is no scenario outside of sexual abuse that would infect your children with HSV 2. And I would never discourage a mother from having a child or more children due to HSV 2, no. As long as the provider caring for the mother during a pregnancy knows about the herpes and is handling it.

      How exactly were you diagnosed with HSV 2?

      Hi Terri, I was diagnosed 12 years ago visually (my outbreak was horrendous) and just recently my new provider did a blood test which came back positive for HSV2 antibodies ( no surprise)

      I think I have mostly gotten over my fear of infecting my children but I am still SO paranoid about whitlow. I know you say it is highly unlikely to infect ourselves somewhere else, especially since I have such a long standing infection and I am on suppressive therapy, but my OCD and anxiety don’t let me get rid of this fear. I get dermatitis on my hands, especially in the winter, so I feel Like I am extra vulnerable and at an increased risk. As I mentioned I feel the need to wear a glove in the shower, wear a glove when I handle dirty towels or laundry, and just overall am SO afraid of getting whitlow. Besides the fact that autoinoculation is highly unlikely, the ONLY way this could possibly happen is if I rubbed broken skin on my hand in my genitals or a lesion (I would never do this). It seems the only way it could happen would be indirect contact.

      I am having a hard time having HSV even after all these years! I am working closely with my OBGYN right now and it seems like in the past I frequently assumed I was having an outbreak when I was not. When I go to the doctor when I have these symptoms that I always associated with an outbreak, she has never seen a lesion or anything suspicious. But it is so hard for me to accept that things are much better than I had previously thought for some reason!

      Lastly, do you think there will be better treatments any time soon?

      I would strongly suggest that you work with a cognitive-behavioral therapist to address this issue. It’s getting in your way and it is a treatable concern.
      You know that this is an irrational fear – you have well-established infection, you are on suppression and what you thought were outbreaks often are not.
      I can’t really offer much help beyond this advice, I’m afraid

      Hi Terri, I am working with a therapist and I have been making progress but still struggle a lot. As a reminder, I have had HSV2 for 12 years, unfortunately passed it to my husband too. The birth of my first child triggered HSV OCD. I battled an extreme fear of passing HSV2 to my children (have overcome that fear for the most part) but I still have OCD about contracting whitlow (in my mind, having HSV on my hands would make it extremely likely to pass it to my children)

      -I take valtrex every day and am currently experiencing an outbreak (anxiety trigger). Are breakthrough infections worrisome?
      -I never touch my genitals, but now I am worried maybe the past day I accidentally touched my towel or toilet paper that touched my genitals and it could cause whitlow. possible?
      -It is impossible for HSV to pass through clothing or towels in ANY circumstance?

      It is normal to have an outbreak every now and then while on Valtrex

      Passing HSV to a baby in utero is rare. A c-section would eliminate the risk of transmission at birth,yes

      Some people have more outbreaks during pregnancy, but it varies

      That would be extremely unlikely and I don’t think it should be a worry

      You would need a break in your skin and come directly into contact with virus, correct.

      I think the most hopeful thing right now is gene editing.

      I now find myself 6 weeks pregnant. Pregnancy does not increase one’s chance of autoinoculation?
      -Herpes in utero is not something I should worry about?
      -I am on suppressive therapy and I think it would ease my mind if I continued to do so. I know I need to speak with my provider and you cannot give medical advice however, the literature you have read on valtrex is first trimester does not seem to present a risk to the fetus?
      -I live in the north east and my hands almost always have cracks during the winter from the weather. Does this make you more concerned about me giving myself whitlow?
      -Have you ever had a patient with a longstanding HSV2 infection give themselves whitlow?
      -I never touch my genitals, so it is impossible to give myself whitlow correct? No amount on a towel, underwear, toilet paper, etc. would be enough to infect myself? And you say this with complete certainty?
      -You really truthfully think I should take giving myself whitlow off my worry list?
      -My husband is not as hygienic as me. What advice would you give to him to prevent giving himself whitlow (he is hsv2 positive)
      -About a year after contracting HSV2 I woke up one morning all around my mouth was red, inflamed and there were bumps around it. And my lips themselves felt like sandpaper. My bf at the time (and hsv2 giver) said it is probably herpes bc I gave him oral sex a day or two before. Thinking back I wonder if this wasn’t HSV, what do you think? I don’t remember them being blisters, scabbing, any tingling or pain. And I feel like after having HSV for a year an outbreak on my mouth would be mild. Whenever something similar has happened again my lips feel like sandpaper and then become very dry. Does this sound more like dermatitis to you? I have extremely sensitive skin. I know it’s hard to say, but just wondering your opinion
      -I know you say not to share towels but I have young kids and they touch and grab things without thinking. what is the actual risk here?could hsv survive 24-48hrs< ?

      I would say the risk of in utero infection is very low AND it’s ok that you’ve taken Valtrex during your very early pregnancy.
      Cracked hands do not make me worry more about autoinoculation.
      I have never had a patient with established HSV 2 get herpes anywhere else that they didn’t have it already, no.
      I really do think you can take whitlow off your list, yes, and I’m not at all worried about your husband getting it either.
      The symptoms around your lips do not sound like herpes to me, no.
      I’m not worried about towels that have been drying for a bit and I’m really not very worried about towels in general. That’s something we’ve said for years, but I’m not sure how valid that concern really is, quite honestly.

      I’ve had hsv2 for 12 years. Frequent outbreaks before starting suppressive therapy. Have HSV OCD that I am being treated for.
      1. I am 20 weeks pregnant with my 4th child. I think I should I take intrauterine herpes off my worry list, but I once had a D&C and was having an outbreak due to stress. Could I have infected my uterus during the procedure?
      2. I get SO anxious about giving birth. I have already expressed concerns to my OB and will continue to. But you are an hsv expert. What are the risks with a long standing hsv infection while taking suppressive therapy if there are no sores but viral shedding at time of delivery? Does csection completely eliminate risk or is there still a small chance of transmission?
      3. There is no indirect transmission scenario possible where I can infect my children with hsv2 correct?
      4. I also get paranoid about autoinoculation, esp whitlow. I get uneasy touching dirty towels, I worry about drying myself off or cleaning myself and then touching where my genitals rubbed against and getting whitlow. Not possible, even with cracks in the skin?
      5. Does whitlow ever present in an atypical way? I guess I want to know if it would be obvious. I just worry about infecting our kids.
      6. I recently got covid. I had a low grade fever, nasal congestion, then a sore throat with some ear pain. I noticed a few tiny white dots and white patches in the back of my throat. The office I called said people have reported this with covid infection, they weren’t concerned. I have never had an oral outbreak of hsv2, what are the chances it’s been dormant all these years and manifested this way for the first time ever? Also, when I got my positive covid result I started taking 1g valtrex a day prophylactically to be safe and this appeared day 3.
      7. With a genital infection, can sores appear anywhere besides the boxer shorts area? Do herpes ever appear atypical on thicker skin?
      8. Getting a disseminated infection should NOT be a concern of mine, right?

      That’s not how that works. No, you should not worry about your uterus being infection. the virus lives in the nerves that innervate that entire area but not any organ specifically

      Women who have well established HSV, like you, have less than a 1% risk of transmitting to a newborn at birth. C-section would certainly eliminate the vast majority if that already small risk. you will also be on suppression during the last 4-6 weeks, at higher doses than normally. But I think, given your fear, you should discuss an optional C-section. You seem very worried and that’s not good.

      Absolutely not

      Correct.

      Whitlow by definition is obvious, so no, not in an atypical way that I’ve ever seen or heard about.

      I would say almost certainly the spots were due to COVID in what you describe here.

      Well, people can appear on thicker skin like the buttocks, but that is in the boxer shorts area. Genital herpes stays in the nerve group that supplies that area.

      My OCD makes me panic over worst case scenarios of having HSV. I do receive CBT and am working on my fears.
      1. Intrauterine herpes. How do I stop worrying about this? I have 3 healthy children, expecting #4. What are the chances my baby gets infected in utero, after having HSV2 for 12 years. I see it is rare but has happened. Is there anyway to prevent it?

      2. Whitlow. I didn’t even know about whitlow until about 6 years ago and it triggered major OCD. I get anxious sometimes thinking that I wasnt careful enough before and could have infected my hands. But the truth is MOST people with hsv2 do not infect their hands correct? If I had contracted whitlow in the past, I would know because it is a very obvious infection? Whitlow would not be easily confused with dyshidrotic eczema which I get on my hands sometimes? I shouldnt worry about having an asymptomatic infection of whitlow in my hands?

      3. Spreading HSV around my body. Can water, even a drop, transport herpes? Can water running down from my genitals in the shower infect other parts of my body? If I am washing my genitals and some drops of water spray up onto my breast or my eye could that cause an infection? If I use a towel to dry off water on my inner thigh near my genitals and then dry another body part, could I spread herpes this way? Can urine transmit herpes? If I get some on my hand or it runs down my leg a little?

      4. Erythema multiforme- should this be a rational concern of mine? I have had HSV2 for almost 13 years and before suppressive therapy had 4-10 outbreaks a year. Also, is that concerning that I had such frequent outbreaks? That I have more of the virus than other people, or my body didn’t have good control of the virus?

      5. My kids.Even if I have dampness from sweat or whatever, herpes can not get through clothing? Sometimes my kids hang around my legs and will touch my genital area accidently (with clothes on). Could herpes live in an loofa sponge, towel or washcloth more than 5 minutes or so?

      No, there is no way to prevent it and yes, it is exceedingly rare

      Correct. you should not worry about this. And extragenital or extra oral infection almost always happen during the first infection with either a new oral or genital infection and you are way past that. Whitlow is really obvious, yes

      None of this is a risk of infection, no.

      This is not something you should worry about no,

      HSV does not go through clothing. We recommend that people not share towels or washcloths.

      To reiterate my previous concerns…
      -You truly don’t think I should worry about intrauterine hsv after having it for 12 years? Do you think taking my daily antivirals decreases this risk more?
      -You can say with 100% certainty I cant spread hsv around in the ways I described above with water and drying off?
      -I know not to share towels and washcloths, but my kids have brushed against a towel or loofa sponge that is hanging to dry. Is there any risk there?
      -Erythema multiforme. You think getting this after several outbreaks of hsv2 over the past 13 years would be very rare? Does having extremely sensitive skin or eczema put one at risk? If I were to get it, do the rash lesions body spread hsv? Just SO worried about transmission to my kids.
      -My therapist asks me what upsets me most about hsv. I worry about it becoming unmanageable or spreading it to my kids. You can say with 100% certainty, I cannot pass this to my kids in day to day life. There will NEVER be enough hsv on clothing, surfaces, my hands to spread it to them? I get nervous about any entry point for hsv on them- their eyes & mouth (cleaning their face, brushing teeth), genitals (diaper changes), cuts & scrapes which they have all the time. I shouldn’t ever worry about preparing food for them, the toilet seat, a shared hand towel in the bathroom?
      -My infection becoming unmanageable. Disseminated infection is an irrational fear? Hsv does not move throughout the body and appear somewhere else?
      -Even though I used to bite my nails and cuticles a lot when I was younger, you still dont think I should worry about having contracted whitlow in the past? I def was not overly cautious like I am now, but I knew that hsv is very contagious and to immediately wash my hands if I touched a sore, Im confident I always practiced proper hygiene.
      -Do you think after 13 years I could mistake bug bites for hsv on my leg? I always worry about every bump or itchy spot that appears on my body and I don’t know how to get past it

      Yes, I think you should not worry about this and antivirals may indeed help reduce this risk but I don’t believe we have data on that issue.

      Yes, but remember that with genital infection, you can have outbreaks anywhere in the boxer shorts area or down the leg even into the foot – that is the innervation of that area.

      No

      -Erythema multiforme. You think getting this after several outbreaks of hsv2 over the past 13 years would be very rare? Does having extremely sensitive skin or eczema put one at risk? If I were to get it, do the rash lesions body spread hsv? Just SO worried about transmission to my kids.
      EM is an immune response to the herpes infection and yes, I think it would be very unlikely to show up now.

      Not in the normal health adult, no.

      Herpes lesions on the leg look very different than bug bites. They present as water blisters on a red base and though they may itch, they don’t itch like bug bites.

      I know my questions are repetitive & maybe even tiresome to you but please understand the idea of hsv infecting my kids or causing major health problems for me is very emotionally distressing. I’m trying SO hard to work through it with my therapist. It all started after I had kids & hsv really started freaking me out.
      -have you heard of intrauterine hsv2 in a longstanding infection?
      -Does hsv live in the sacral AND lumbar dermatomes? If both I see hsv can appear anywhere on the legs why is the boxer shorts area always mentioned then?
      -would suppressive therapy reduce the chances of hsv OBs on thicker skin or not necessarily?
      -What signs or symptoms would you consider a possible OB on the legs that should be covered to protect my kids? Unfortunately every red mark or bump I jump to HERPES and want to think logically about it
      -I know the mons pubis is too thick for viral shedding but what about the labia majora?
      -Simple handwashing is enough to prevent whitlow autoinoculation? I don’t have to worry about missing a spot or needing to clean again, correct? Hand sanitizer also kills hsv?
      -I should not be nervous wearing a bathing suit or changing around my kids?
      -My dad has OHSV1. As long as he doesn’t kiss my kids with an active outbreak he can’t transmit it to them?
      -You are 100% certain there isn’t enough hsv on dirty underwear, a sanitary pad, a piece of toilet paper, etc. for autoinoculation or to infect someone else?
      -Dealing with my anxiety: I tell myself hsv is a very well studied virus and I can trust the info from the medical field? As you know, ppl write all sorts of stuff on the internet but this is a very understood virus?

      No

      -Does hsv live in the sacral AND lumbar dermatomes? If both I see hsv can appear anywhere on the legs why is the boxer shorts area always mentioned then?
      The sacral ganglia, but if you look at a nerve dermatome picture, you will see that it also innervates the back of the leg into the foot. The leg skin is too thick for shedding which is why it is not a concern.

      -would suppressive therapy reduce the chances of hsv OBs on thicker skin or not necessarily?
      Everywhere, yes!

      -What signs or symptoms would you consider a possible OB on the legs that should be covered to protect my kids? Unfortunately every red mark or bump I jump to HERPES and want to think logically about it
      I have seen herpes on a leg below the knee maybe twice in my whole career. And it looked like a group of small water blisters on a red base.

      -I know the mons pubis is too thick for viral shedding but what about the labia major?
      Yes, that could shed
      -Simple handwashing is enough to prevent whitlow autoinoculation? I don’t have to worry about missing a spot or needing to clean again, correct? Hand sanitizer also kills hsv?
      Simple handwashing is adequate yes. The soap destroys the lipid layer that surrounds the virus and the virus falls apart and is rendered harmless.

      -I should not be nervous wearing a bathing suit or changing around my kids?
      No, that’s fine.

      -My dad has OHSV1. As long as he doesn’t kiss my kids with an active outbreak he can’t transmit it to them?
      Correct

      -You are 100% certain there isn’t enough hsv on dirty underwear, a sanitary pad, a piece of toilet paper, etc. for autoinoculation or to infect someone else?
      Yes. Obviously, if you wipe a sore with a pice of toilet paper and some picks up that exact same toilet paper within moments and wipes their genitals, there is a remote chance that transmission could occur but I am assuming that you flush toilet paper down the toilet after using it.

      -Dealing with my anxiety: I tell myself hsv is a very well studied virus and I can trust the info from the medical field? As you know, ppl write all sorts of stuff on the internet but this is a very understood virus?
