Hello! 2 weeks ago I (male) had unprotected oral and vaginal sex. I got my recent lab results back and I am positive for HSV1 with an index value of 32. Throughout my life, I don’t ever recall having an outbreak on my penis and can recall having a few canker sores inside my lip. My brother also has OHSV1 since he has had cold sores as a child. I have lived with him for 20+ years. Due to all of these facts, I am assuming I also have OHSV1 since before two weeks ago, I can count on two hands the times I’ve received oral sex and, most times were protected. I am going to wait 12 weeks post exposure to retest for HSV2. The female I was with 2 weeks ago mentioned she had cold sores in the past but hasn’t gotten one in years nor did she have one when I last saw her. I don’t have any blisters on my penis just some tingle sensation on my penis head and some slight redness. 2 days ago a physician said it could be herpes but they weren’t sure. So they tested me for herpes (2 weeks post exposure) and I received the results as listed above. I visited a dermatologist today and they said it was not herpes but ‘Balanitus’, and what I thought could be blisters was just ‘pearly penile papiles’. However I am a bit concerned because my anus region can become a bit irritated, sensitive and lubricated after wiping. dermatologist said it could be hemorrhoids but I would have to confirm with a GI. I have no flu like symptoms, no swollen nymph nodes, no pain, But I am just very paranoid. They have me some medicine to apply to the head, I will try it out and let you know how it works. However, I was under the impression that once you have HSV1 in one location (for example in my case orally), it wasn’t possible to get it in another area of your body (in my case, genitalia). Is this only when talking about your own body? From my mouth to my genitalia? How can I know if I have GHSV1 if I have no real blisters to swab? I would like to know your input on this situation. Thanks!

I would trust the dermatologist on this one, for sure. Your HSV 1 result is likely the result of an oral infection but we cannot know with 100% certainty. I seriously doubt if you have it orally that you would then get it genitally.
HSV is almost never the cause of simple balanitis – usually that is fungal/yeast.
And pearly papules are very different than blisters – PP are normal and very common and are solid – blisters are fluid filled. They are entirely different.I would believe that you do not have genital HSV 1
