Male 42 yrs old.Diagnosed visually by dr. at 21 yrs old. No blood test.1st OB appeared to be HSV. Cluster of small blisters at base of penis on top by near pubic hair (~ 1/2″ patch). About 3-4 more OBs in next 4-6 months with same appearance/same place and painful symptoms so started daily Valtrex.OBs became fewer and further between but the pain/itching at site of the Obs would come and go with some frequency. So only a couples Obs in my 20s.About 31 got a single blister at base on top near pubic hair(football shaped ~ 1/4″ long). It lasted about 4-5 wks).This appeared a day after I ate lots of almonds).After this OB I had lots of pain in the penis in general along with pain at sites of my original OB and the new location. Pain would come and go lasting anywhere from 1 day to over 8 weeks. around 33 yrs old, had a small growth I thought may have been a wart on the side of the shaft half way down.The site of removal turned into another OB from the trauma I guess.Pain in penis has been on and off since the OB at 31.General Pain in shaft, itching/tingling at sites of OBs, general pain in pubic region and some lower back pain/ aches in legs.No doctors have been able to diagnose reason.They put me on some antibiotics and also sent me for me for ultrasound on penis. Nothing came of it.I have not had a visible OB in about 10 yrs, but the constant pain, tingling, general pain in pubic region is ongoing for weeks/months at a time and then I have periods of weeks/months with barely any pain or other symptoms.It just generally feels like I am on the verge of an OB lots of the time.I have had several long relationships with unprotected sex and as far as I know I have not transmitted it to anyone.I am frustrated and my doctor and urologist just think it is related to the herpes, but the length of time does not make sense to have prodrome for these extended periods of time.I avoid L Arginine foods,take vitamins.This has been almst dly for ~5 mnths this time.

I personally don’t think it makes sense that this would be due to herpes. Herpes is far more intermittent than this. And it doesn’t cause pain into the penis, normally. It causes skin pain with some outbreaks. Have you ever actually been tested to know if this is HSV 1 or 2? Have you had a thorough prostate evaluation?

I have had prostate exam that was neg. I just rcvd IGG test results pos for HSV2.6.5 was result. Does higher number indicate anything about my situation? IGM was neg. I agree the symptoms lasting so long do not add up, but primary site of the pain is usually at the point of the 2 sites of OBs. Sometimes sharp, other times very dull general at the sites of OBs. Could it be post herpetic neuralgia? Or almost constant prodome? It has been improving over past couple weeks, but not gone and has been going on for months. It has done this in past for months and then improves for a period of time with intermittent symptoms of pain, so I am sure it will come back for prolonged period.I had been on 1000mg Valtrex daily for 15-20 yrs. I just changed to 800mg daily acyclovir a few weeks ago thinking that changing med may help. Ever heard of the Valtrex becoming less effective over time? Or some ppl responding to one better than the other? Is acyclovir as effective at reducing transmission as Valtrex? I see Valtrex claiming reduced transmission but don’t see that for the acyclovir, online at least. Since I have had this over 20 yrs, am I less contagious? Doctor is sending me to Infectious Disease doc for consult regarding the pain. Thought on this? Is there another doctor you may recommend? This constant pain is driving me crazy. It makes me not want to have any sex or even start relationships. But as mentioned above, I have had several longterm relationships with unprotected sex and nobody has gotten it that I am aware of. Prodrome is normally time limited as is post herpetic neuralgia, but I cannot say with 100% certainty that this is not herpes related in some way. Valtrex is the only drug that has been studied for reduction of transmission but many studies have looked at viral shedding on both drugs and they are about the same when taken has directed so we can extrapolate that they also reduce transmission equally as well. You are likely far less contagious since you have had this 20 year, most people do shed far less over time but it never goes away completely. I think the only thing the ID is going to do it talk about antivirals, but you’ve done that already. You could consider taking 500 mg twice a day vs. 1000 once a day and see if it makes any difference in your symptoms. Sometimes that helps.
