24 yr old male. Mid-March 2018 I went to Tijuana, Mexico for a bachelor party. Long story short, ended up at a massage parlor and engaged in a threesome with two female CSW. Each performed protected oral and protected vaginal sex on me, changing condoms between each act and each girl. I’ve been HSV1+ since childhood but only get a blister once every 2 yrs. Recently met a girl and don’t want to do anything until I am in the clear.
I’ve not had any “classic” lesions or constitutional symptoms yet. At 10 days I had a tiny pink/red, painless spot at the base of my penis on the top side. It resembled a nick one would get while shaving their face. I never saw a blister (I’ve been checking every 5 minutes since the encounter and take meds for anxiety). The spot faded away without scabbing or crusting 5 days after it appeared. There were no more “symptoms” until 6 weeks. At that time I developed 3 fissures in the inguinal folds of my groin. 2 on the left and 1 on the right. Again, I did not see blisters before their appearance. I believe that these may be tears in the skin due to repeated/constant pulling/handling while checking the area. These fissures disappeared within 6 to 7 days and were only slightly uncomfortable. Had several minor rashes and “pimples” on face, chest, neck, buttocks and thighs (inner and outer). These stand out to me as I normally never get pimples at all. The rashes only last a day or two each time and move around. Every bump, itch, red follicle sends me running to the bathroom to take butt/crotch selfies. I am terrified that I may also auto-inoculate myself all over my body with HSV2 if I am positive and have not built sufficient antibodies.
I had a 10 Test STD panel and trich test at 8 weeks: All came back neg. except HSV1 (IGG 48).
1. How risky would you rate this encounter?
2. How reliable is my neg. 8 wk HSV2 result given the exposure and symptoms described?
3. In my case, would you recommend another test at 12 wk or just move on? -

I think the encounter was well protected, good job!
Nothing you have said particularly worries me except perhaps the “nick” but who knows what that could have been. We do recommend a final test at 12 weeks, yes, thought there is probably an 80% chance you will stay negative. The igG test misses 8% of hSV 2 infections, compared to the western blot but this encounter was very well protected and I think presents very little risk of anything.Terri

I figured out the “nick”. It was a thin vein. Saw another one and got a close up picture. Not any kind of lesion. I also got my 12 weeks HSV 2 results back with a .34 (NEGATIVE)!!!! Yay!! Given my exposure and “symptoms” combined with this 12 week result, would you say I am in the clear? No more testing needed? Please let me know as I am anxious to resume my life and quit worrying about this possible exposure.

Yes, I think you are fine. The IgG test misses 8% of HSV-2 when compared to the gold standard Western blot but we need to take into consideration that your contact was well protected when we look at those statistics. Given the whole picture, I think infection is extremely unlikely in your situation and I believe that it is reasonable to assume that you are uninfected
