Hi Terrie,
I have had cold sores since I was a child, so I already knew I have hsv1. I did get tested in 10/2021 and my results were positive for hsv1 IgG 45.00 and negative hsv2 IgG 0.06.My ex husband prior to 10/21 had outbreaks and blamed me for them, saying my oral cold sores gave him genital sores. His outbreaks were really severe. I blamed myself for awhile for it. We have had unprotected sex 3 times max after his outbreaks. He was on Valtrex, but idk how consistent he was with it. I also don’t know if he had hsv1 or 2. Since 10/21, I have had two sexual partners, both unprotected and I have been on Valtrex for at least a year (500/daily). I have never had genital sores or symptoms. 3 weeks ago my anus was itching and I scratched it in the morning half asleep. It felt uncomfortable after I woke up so I checked and found two flat surface red, looked like cracked skin around my anus. It didn’t have a blister, wasn’t in a cluster. One of them did look like it was wet, but idk if this was because of my scratching or what. The day before that or the same day, I can’t remember for certain, I had flu like symptoms, swollen lymph nodes on my neck, fatigue, sore throat. I also had mouth sores in my mouth. They looked and felt like canker sores, but there was clusters of sores inside my lip, barely under the skin of my lip that looked like cluster of cold sore but just inside my lip. I know this is unusual.
I took extra Valtrex to treat this and immediately after had chills and nausea. The two sores around my anus felt uncomfortable, but they were not super painful or itchy. They disappeared 2 days later. No scabbing, just disappeared. I continued taking Valtrex daily and 2 weeks later I found another sore around my anus (different spot). It disappeared after a day. I got tested today blood test, and waiting. 1. what are the chances this can be hsv2 from a past exposure? If there is a possibility, why would my initial value be so low for 0.06 for hsv2?

2. Could this be hsv1 appearing genitally now that I have had oral hsv1 since childhood? What are the chances of this?
3. When I got the labs done this morning I didn’t take my daily Valtrex, but I have been on them for over a year daily. Will this interfere with results and if yes, is it more likely to show a false positive or false negative results?
4. Would you recommend getting WB done? Should I get off of Valtrex and how long should I wait?
Again, I have not had any new sexual activity since September. If I contracted hsv2 then, would it show up now as the first outbreak?I know these are a lot of questions. I appreciate your time.

Results came back.
HSV1 IgG 45.50
HSV2 IgG .10
These are almost identical to the numbers from testing in 10/21. Is it safe to assume the genital symptoms I had were likely due to something else since I have had cold sores my whole life and never genital symptoms?I remember reading that once you have it orally it is unlikely to acquire it genitally? My doc says it is uncommon but not impossible.
She also said Valtrex wouldn’t have affected my results. Reminder last sexual activity was back in September.
Another question I have is if a future partner is OHSV1, are we safe to have oral sex without worrying to transmit it to each other as ghsv1?
I have not heard from you since my first post, so idk if I am doing this incorrectly or if I need to pay more. Please respond so I know what needs to be done.

glad you had results to post before I answered.
I would say that this says you don’t have HSV 2 genitally and that HSV 1 genitally is highly unlikely. I would agree with your doc – transfer to a new location after established oral infection is extremely unlikely but possible (I have seen it once in 40 years of practice and even that one, I’m not positive about)I think this also answers your question about oral sex with a new partner who also has HSV 1.Terri

Thank you for your response. I have some follow up questions, please let me know if I need to pay again.
1. I still have anxiety about having ghsv1. This could be somatic, but I have this strange feeling around my anus (the location where the red bump was). The closest way I can describe it, it feels like a tingle or something crawling under my skin. It isn’t constant, but comes and goes. I keep checking but I don’t see anything.
2. Do you think I should do WB?
3. What could my initial symptoms possibly be if not ghsv1? All the other STIs came back negative.
4. Is it possible to transfer the virus to my genitals if I used my spit to masturbate?

No, not unless there is some chance someone you had sex with has HSV 23. What could my initial symptoms possibly be if not ghsv1? All the other STIs came back negative.

We we’ve discussed here that would be highly unusual.I can see that you don’t really believe that you don’t have HSV 1 genitally so the only way to know is to continue to have swab testing done of the area to be more certain. A western blot will pick up your oral infection and it won’t help you really.Terri
