3 months ago I tested positive for HSV 2 with a result of 2.95. I did not have any sexual contact for 3+ months prior to that test. Is there anyway this could be a false postive? I have not had any symptoms in 7 months since my last encounter. My HSV 1 was negative.
There is probably a 35% chance that this is a false positive. So yes, there is a chance. You need confirmation by western blot to clarify this index value.

Is it possible to have an outbreak and not realize it? Also after taking the western blot how long does it take to get results?
Yes, it is possible to have an outbreak and not notice it, particularly if you have HSV 1 already.
From blood draw to results with the western blot is about 2-3 weeks.
