My boyfriend cheated on me with another woman around March 3rd. He made out with her and she gave him head. That was it. He does not know her at all and has no way to contact her. He has had absolutely NO signs or symptoms of possible HSV, no breakouts, NOTHING. He went to urgent care a week or so later for what ended up being a yeast infection, however they did a full STD work-up.
The results showed:
HSV 1 IGM Negative
HSV 2 IGM Positive <1:20
HSV 1 IGG Negative <0.90
HSV 2 IGG Negative <0.90He re-tested 9 or 10 weeks later (I’m thinking it was 10) after the hook-up with the girl. The new results showed NEGATIVE for everything:
HSV 1 IGM Negative
HSV 2 IGM Negative
HSV 1 IGG Negative <0.90
HSV 2 IGG Negative <0.90Do you think this is accurate as it was only 10 weeks after exposure? Does he definitely not have HSV? Should he re-test? Should we be scared or is he in the clear? Thank you.

The test he took picks up 70% of HSV 1 and 92% of HSV 2 infections. If he has had no symptoms at all, it is very unlikely that he is infected. Has he ever in his life had a cold sore on his lip or in his nose?

He has never had a cold sore on his lip or nose ever in his life. He had what was considered a yeast infection and inflammation of the urethra (treated with antibiotics) but no other signs or symptoms.

It’s difficult to know from this test if he is infected with HSV 1 for sure, but if he got brand new infection from this contact, I suspect he would have had symptoms that were noticeable.

What about HSV2? It was HSV2 IGM that was originally positive and then turned negative? Do you think he is in the clear for that? The urethritis came about a week after the contact with the girl. Is urethritis associated with HSV2? It did clear with the antibiotics and then there was no other symptoms or signs after that. Do you think it was too soon to tell? Should he retest? I’ve read the IGM test is useless, is this true? Also, how often can someone get HSV2 from receiving oral sex? Thank you.

Great question. The IgG test misses 8% of HSV 2, but if the only real contact with him receiving oral sex from her and kissing, the risk here is HSV 1, very very little risk of HSV 2.
HSV urethritis would normally be associated with external lesions as well. he could have acquired other bacterial STD from her, like mycoplasma,if they did have intercourse and that would respond to antibiotics. I supposed he could also have picked up a bacteria from her mouth as well though that is less likely but still possible. A yeast infection is fungal, not bacterial, would be unlikely to respond to antibiotics, so that’s curious.
The IgM is quite useless and the Center for disease control recommends against ever doing test to try to determine if an adult has herpes or not. It has many false positives.
We like 12 weeks for a final test from exposure but 10 weeks is pretty darn close.
