I tested in 9/2020& 3/2019 negative with an IGM for herpes 1 &2 in annual testing. I tested in 3/2019 because of breaking up with my ex and I tested in 9/2020 to be sure after only being with one primary partner after my divorce.
I tested in 7/2021 & 9/15/2021 for a double check as positive. I’d only had sexual intercourse with my same primary partner but he has a blood disease that makes him have platelet shortages and gets transfusions and infusions. Could he have gotten HSV from the transfusions? I know they don’t test for HSV when you donate, and there are articles saying it’s theoretically possible.
What was the index value of your positive tests, please?

Are these values for IgM testing? You don’t list any IgG testing.

One would not acquire herpes from a transfusion, no. It is a sexually transmitted infection. An index value of 21 for HSV 2 is a very sound positive and should be believed.
