      Hi Terri-
I had unprotected sex in May of 2014. In June of 2014 I was tested for all stds including Herpes and came back positive for HSV1 in the low range (don’t remember exact number). I had never had a cold sore and my doctor said a majority of people have it and i could have had it for years and not to worry. I have not been sexually active since and in July of 2015 i had sores on my vagina. I immediately went to see my gyno and she squeezed and took a sample from the lesions and the lab results came back as hsv1. i have never had another outbreak since. two weeks ago i had protected sex with my new boyfriend and i had some light bleeding. i went to a walk in gyno (not my gyno) just to be sure all was ok. she examined me and said all was ok, bleeding was likely due to size. she did bloodwork and the IgG test came back with HSV2 3.3ai. does this mean I have HSV2 even though my gyno had tested the actual sores (one occurrence) and determined it was HSV1? i just want to determine how best to protect my love one as I was told suppressive therapy wasn’t necessary as long as i use condoms and monitor any additional outbreaks which i understand to be minimal if at all with hsv1.

also let me know if an additional blood test would provide greater clarity.

This does not mean you have HSV 2 but it certainly could mean that. The 3.3 index value that you have suggests a positive, but we do see false positives in the range of 1.1 to 3.5. I believe that you should obtain a herpes western blot, the best confirmatory test available to us for clarifying these low positives. Our clinic can order that to be drawn at a lab near you. You need to know for sure. While having both HSV 1 and HSV 2 genitally is not at all common (about 5-6% of people have both genitally), it is possible, and if you are truly positive for HSV 2, then we would assume that is genital. I know this must be a bit overwhelming for you right now but let’s get the diagnosis correct and then see what to do next. How does that sound to you?

Please let me know what other questions you have.

      Terri-
Thank you for helping clarify. You said I need to know for sure. I have read conflicting information about the difference between type 1 genitally and type 2 genitally but curious what it would mean for me if i am in fact hsv2 vs 1 genitally. appreciate your help.

in other words what is the real different between the two if located genitally

HSV 1 genitally recurs infrequently and is shed infrequently – recurs about once every other year on average and is shed on about 15 days of the year. HSV 2 genitally recurs about 4-6 times per year on average and shed on about 13 out of every 100 days, so big difference between the two in the genital area.
