If my partner is positive for Hsv1 and she only has ever had outbreaks on her genitals, and never anything on her mouth, is it safe for me to kiss her on the lips and for her to perform oral sex on me?
If your partner only has a genital infection, then yes, it is safe for her to kiss you and for her to perform oral sex on you.

How can you know if she only has genital infection. She only gets sores on genitals and has never had any on her mouth. Is there any way to know for sure.
That’s a great question and there is no way to know with 100% certainty. However, HSV 1 strongly prefers the oral area over the genital area, so I think if she was infected orally, she would likely recur there. that is more likely than genital recurrences, actually. Given that half the population has HSV 1, 70% don’t know it and the standard test misses 30% of HSV 1 infections compared to the western blot, it is truly a crap shoot when someone says “I don’t have the cold sore virus”. At least, with this person, they know and can do things to reduce transmission to a partner, like taking daily antiviral medicine, as an option
