Hi Terri. Thank you for taking my question. I’m a 44 year old male. Right at about 6 months ago in early October of 2022 I regretfully paid for a nuru massage from a CSW while on a business trip in Florida. There was no sexual penetration but there was some contact of her vagina to my penis at times while she was going through her massage of me but again no acutal penetration. A few weeks afterwards I began to feel the regret of doing this and started to think I may have contracted something. I have been tested for every STI out there besides HSV1 or 2. Fast forward into late January I started noticing the meatus or tip of my penis was a little irritated or red and has remained this way for about 2 months now. I have been prescribed numerous steroid creams, anti-fungal creams, and diflucan for yeast. None of these have really helped. I have been having frequent urination, and some dribbling after I urinate (however I have been doing this for a few years now) which I am told by my GP that it can start to happen to some men in their mid 40’s. The head of my penis has been a little more sensitive as well since late January. I have not had any typical herpes lesions or bumps anywhere but since late January I experience at times some tingling in my feet and such at times. I have had some red jock itch looking lines on both sides of groin area where groin meets upper thigh/scrotum area but my GP attributes that to being active and sweaty. These lines pretty much remain slightly red all the time and not just come and go. Over the weekend I played golf 2 days in a row in pretty hot temperatures, I was wearing some boxer briefs that I feel began to irritate that area but also noticed some tingling in my feet over the weekend as well. However getting out of the shower this morning I noticed and felt a whitish single clear fluid filled blister on the left underside of my scrotum in that groin area near the reddish jock itch irritation. Could this be my first HSV2 blister??

I should also mention that as soon as I touched the small blister it popped and some clear fluid ran out of it. I put some neosporin on it and it doesn’t seem to hurt. Would this really be a single herpes blister outbreak 6 months after possible exposure?

Your sexual experience presented a very little risk for the acquisition of HSV-2. Herpes does not cause an increased frequency of urination. It does not cause a skin change like redness for two months in a row. It does not cause dribbling after urination. It does not cause a rash on both sides of the thigh. Most of the time, when people report tingling or pain into their feet or back, herpes is not identified as a cause. I don’t know about the blister on the scrotum, but the scrotum is an unusual location for lesions of herpes. If you get another lesion like that, I would strongly suggest you have a swab tested immediately. All in all, to me, it doesn’t seem like herpes is likely in your situation, but I have no idea about your previous sexual history and risk for acquiring herpes before this encounter.
