      Terri,

I recently had a comprehensive bloodwork panel done, in connection with fertility planning. My HSV-1 IgG test came back 1.47, which identifies >1.09 as “positive.” My wife and I have been together, monogamous since 2010. I am over 40. I have never had symptoms, and to my knowledge neither has my wife.

I understand this is a very low number, a likely candidate for a “false positive” result. My question is how likely is it a false positive? And what follow-up test can I do in order to confirm the accuracy of the results? If it is indeed positive, is this result (1.47) consistent with exposure some 10 years ago, either from my wife (then girlfriend) or prior thereto; otherwise, I am completely perplexed as this cannot be a new exposure.

We don’t have good data about HSV 1 and false positives like we do about HSV 2. What we do know is that everyone who tested at an index value of 3.0 or higher for HSV 1 confirmed by western blot. Clearly, you are not there yet. But about half the US Population has HSV 1 so I would not be surprised if you were truly positive. You could do a western blot to confirm this if you really want to know for certain about your HSV 1 status. And remember that most often, this still isn’t usually an STI.
