Background: Diagnosed in 1996 with HSV2 by swab. More recent blood tests were positive for HSV2 antibodies, negative for HSV1 antibodies. At first (1996-97) I had many outbreaks, about 6 weeks apart, always on right buttock. Gyno visits during early outbreaks did not identify any vulvar, vaginal, or cervical lesions. After the hormonal upheavals of peri-menopause, it settled down to 1 or 2 OBs per year.
Current: I’ve been under extreme stress the last three months–two very elderly parents with overlapping health crises, hospitalizations, rehab, and I’m the only one of the “kids” (all in our 60s) in the same state as our parents. Since this started, the last week of October, I’ve had 4 outbreaks in that short space of time, the last one started this past Sunday evening with red prodrome in usual spot. I’ll see my PCP Friday and talk with her about suppression therapy.
Question (finally!) My question is this: What is the current recommended suppression dosage? One 500 valacyclovir or two 400 acyclovir daily? Is there research evidence for some period of suppression to “calm down” a series of frequent outbreaks after which the daily suppression is no longer needed?

I would recommend that you start suppression with 500 mg of valacyclovir and increase it to 500 twice a day if that is not adequate or you could go to 1000 once a day.
Sometimes, taking these suppressively for a few months does seem to calm things down enough to stop but I see no reason to stop. Maybe get your folks through their lives and illnesses and then think about stopping. The meds are safe and effective.

Thank you Terri. My PCP prescribed 90 days of valacyclovir @ 500 day, saying go to 2x a day if that doesn’t fully suppress it. Pricier than acyclovir under my Medicare Part D, but not a big deal–the pharmacy had a coupon to cut the cost from just over $100 to $48.
