Nurse Terri good afternoon :
I posted a question on no November from last year. I described the experience I had with this lady, mentioning that I never got to have intercourse with her nor our genitals got to touch each other. Found out later that lady has HSV2. The only thing I did was placing my scrotum on her outer left thigh ( Almost buttocks). I am trying to find that post so I could re post from there but I cannot find it. Here’s the thing Terri: This last February 5th ( 4 months after that exposure) I took a test and came out negative. I Had also mentioned that I was HSV1, and you told me that 4 months would be time enough to wait to test.
1. Terri, would you consider this negative result conclusive then? I have asked in other forums, and they also tell me that 3 months is %90 conclusive.. But that 4 months is even better. That 4 months is enough time for antibodies to develop. But still I needed to confirm with you. Based on your experience, Would you consider rare/ uncommon seeing people, that eventhough tested negative at 4 months after exposure, would test positive afterwards?
2. A couple of days ago, after taking a shower, I was checking my scrotum just as a routine, and one tiny follicule was itching a little and I pressed my nail in it. Next day there was redness around it and gets irritated and stings when I walk. 2 days already and just wound. No blisters. Stings when put alcohol. could this be HSV2? -

Terri I am trying to send a pic… I hope I can reduce the size. But the wound looks kinda yellowish, like if it is trying to scan already. It has been 2 days. Is this HSV 2 like? There are no blisters … Just stings when I put alcohol in there.

That’s the bottom area of my scrotum. And I also meant to ask you… Would that be consider a typical, common area for OBs to show up? I have understood, and have seen diagrams that show that OBs show up in areas that are involved in several minutes friction during sex like penis shaft, and upper scrotum ( which gets to touch labia on women).

The pictures you are sending are very blurry. If you have a lesion that is open and looks concerning PLEASE go and have it swab tested now. Could this be something that you scratched with a fingernail?
I would say if you waited 4 months, that’s enough time for as reliable a result as you are likely to get.

Sorry about how blurry are the pics. And I am so concerned right now that I forgot about condensing all my concerns in one post as much as possible and avoid sending so many.. My apologies. The pic shows some redness with a tiny wound already showing scab. This was something unexpected and I would love to hear your opinion about it. I’ll explain in more detail : 3 days ago, after a routine self checkout, I accidentally scratch a follicule on my lower scrotum. Next day there was some redness surrounding a tiny wound that would STING WHEN EXPOSED TO ALCOHOL( I put a little on purpose to see if it was a wound). It has been 3 days and has now a little scab. Never looked like a blister. Went to doctor yesterday and a soon as she looked at it concluded that wasn’t herpes. She said it didn’t looked like it and that it always manifest itself through BLISTERS.
I have read testimonies of some people who have HSV1 and 2 ( l have HSV1 myself) who say have very mild HSV 2 OB’s. So mild that won’t even develop into blister or bumps. They see just tiny irritated ‘cuts’ and claim they clear fast.
1. How common is seeing HSV2 OBs that manifest this way? So mild? Mainly in people who also have HSV1? Can a HSV 2 OB manifest this way as a tiny wound already showing a scab all over a period of 3-4 days without hadn’t shown no blisters?
You see my concern? I know I already have a negative result 4 months after exposure. And that relieves me a lot… Your words also give me confidence. But now with this thing I just noticed on my scrotum ( 10 days after my last test) and after reading those testimonies, fear is coming back again…
Please if you don’t mind sharing with me your opinions about this scenario.
Now I can’t stop thinking this is a HSV2 OB manifesting itself so mild due to my HSV1 condition.Also that area show on the pic is the bottom area of my scrotum. And I also meant to ask you…

Yes, HSV 2 can appear this way. Not all herpes presents as blisters. At some point, blisters break and then the outbreak can look like many things. But your doctor looked at it and said it didn’t look like herpes, did she swab test it? I think you just scratched yourself and are freaking out unnecessarily.
No, scrotums are not at all typical areas for outbreaks. I seriously doubt that this is herpes.

That tiny wound closed within 4 days and it has left what it seems a swollen follicule. Honesty I am pretty scared before I had read in a post, that a guy had also tiny ‘cuts’ that never developed into blisters and healed within a few days. He was also HSV1. And said he got this swabbed and showed it was HSV2. Obviously I don’t want to think my situation is similar, but is hard to avoid thinking about that. Nor I know at which area this guy developed these ‘cuts’. Mine was at the lower area of scrotum remember? I just kinda softly brushed it off for a few seconds on her thigh/buttocks area. Based on your expertise:
1. For people with HSV1 who are about to experience first OBs, how common is to see people experiencing very mild OB’s like tiny ‘cuts’ that never develop into blisters and then healed within days? Is this common on people with HSV1? Or would you still consider it uncommon, being normally expected a full blown OB? How often do you see so mild initial OB’s on people with HSV1?
Also Nurse terri, just wanted to share with you a couple of pics… To kinda let you know where exactly had contact briefly with this lady ( the white dots mark he point of contact). On the second picture you will see the sacral nerves that are involved in the groin area.
In the pic I see that the nerves are present in the inner area of thighs… However:
2. Do you think this sacral nerves are also present in any random area at the area where I placed my scrotum? ( by sending the pics I wanted to give you a better idea of what happened to me, and also will give me more understanding of if sacral nerves might have been present at contact point)
3. Would you think this is risky? Just touching her briefly with svrotum at that area?
Four months after exposure show negative results given such a low risk experience. But since that tiny wound showed up, I started getting worried again. I keep thinking it might be an OB and my HSV1 status is making mild.

Sometimes we see big outbreaks sometimes minimal ones, we see it all. And I suspect that minimal outbreaks may not even present to us at the clinic.
The virus is not shed without an outbreak in the area in the picture. I am well aware of the innervation of the sacral ganglia, thank you. The point isn’t whether nerves go there – most certainly they do- it’s whether the skin is too thick for virus to be shed from there and yes, it is too thick to be shed from there.

I was just wondering if you have ever seen OBs growing at the area of outer tighs almost reaching buttocks (refer to the pic I previously sent you… I had marked white dots on point of contact) Have you ever seen OBs grow there?

This will be my last question. On this next pic, have you ever seen OBs growing in the white encircled area? Is it uncommon seeing OBs there? ( There is where I had brief contact)

Outbreaks on the buttocks? yes, I have seen outbreaks on the buttocks. Did she have an outbreak there? Remember that that area of the body does not shed virus without an outbreak.

And how common are these OBs on buttocks vs. presence of OBs in other areas? Do you see these often at this area? ( any percentage that you can give to have a. Idea perhaps?)

Outbreaks on the buttocks are quite common, actually. I would say that 15-20% of people have them there regularly

I see.. But see that’s what I can’t still quite understand : You mention that thick skin doesn’t shed the virus. So how do few people still get OBs there? Perhaps because they had broken skin at those areas that came in contact with membranes or OBs from other infected person?

No, they get outbreaks there because the nerve group that supplies the genital area also supplies these areas and all of those nerves are “infected”. So when the virus wants to come to the surface of the skin it can certainly travel along that nerve to get to the surface of the skin. The virus does NOT need to come out the way it went in. I can see why you were confused now.

On my previous post I had mentioned I had a tiny would that felt irritated and healed within 4 days on my lower left area of my scrotum ( looked more like an irritated follicule and the area where it was located would get constant friction with leg when I walked) . 3 weeks after this tiny wound would show up I took another test for HSV2 ( this would be my 5th test after exposure. Haven’t had another encounter since then) and came back negative.
If this had been HSV2, do you think taking the test 3 weeks after the presence of this symptom (that tiny irritated wound) would have shown a positive result?

Let me rephrase that question…
Would testing three weeks after possible presence of symptoms, in this case an OB, give me an accurate result of whether there is a presence of antibodies or not?
Does having symptoms imply presence of antibodies? Or can there be a gap of time, between the moment OB showed up until antibodies show up.
I hope I am not being confusing. That tiny wound worried me and my results came back negative 5 months after exposure. After doc gave me results, she said no need for further testing and that even with my HSV1 status, this results are conclusive for HSV2. She says that under very unusual circumstances antibodies production is delayed at this point..

It isn’t the time from the development of a lesion to antibody testing that matters. It’s the time from the initial infection to testing that matters. I would agree with your doctor that you don’t need further testing for HSV 2. Also I don’t recall ever seeing someone with herpes who only gets lesions on the scrotum.

Thanks for clarifying that. I’ll stop getting tested if you recommend so. Is just that the presence of that tiny wound is kinda stressing me a lot and creating doubts in me even when results show negative after 5 months. I suppose I just wanted to rule out the possibility that that was an OB.
Perhaps it was my fault… What I really was trying to ask is:
For someone that has HSV2 and is experiencing an OB for the very FIRST time, no matter at what point of time after initial infection, Will that person have a presence of antibodies in his blood by the time he experiences that OB? Is an OB a ‘confirmation’ let’s call it, of antibodies presence? Or can antibodies appear after OBs?

Normally, if a person is having a true first outbreak, he will not have any antibody for a while. A positive swab test and a negative antibody test for the same type indicates first infection. An outbreak is NOT a confirmation of the presence of antibody, no. Yes antibodies can appear after an outbreak.
If you are still worried about this, how about if you take a western blot and allow that to be the final opinion on whether you are infected or not?

You’re still not understanding what I’m saying. The timing to making antibody is NOT related to an outbreak, it is related to time from infection with the virus. So if you had a concerning sexual encounter, you would time the antibody test from the time of the encounter, not from the time of any suspected outbreak. Does that make it clearer?
