Hi Terry,
Existing customer here–thanks for everything.
I’ve explained my “exposure” to you in person but understated one thing. Along with fingering a woman and having her grind her wet vagina on my thigh, I also later spit into my hand and masturbated my own penis briefly, possibly transfering her oral bacteria (we had kissed) or vaginal bacteria (from fingering) to my penis/urethra.
I’ve tested for all standard STDs at 8 and 12 weeks and come out negative–all conclusive except for herpes, which is about 90 percent seroconversion rate and only gets to 97 at 16 weeks.
I have tested negative for trich (9 weeks) and will get mycoplasma genitalium results tomorrow (private test).
1. You have told me before I had no risk. Does this additional exposure (via my hand) present a risk or am I just being crazy again?
2. Also, assuming I come back negative on MG test tomorrow, and with my history of clean urine dipstick tests, urinalyses, urethra swab with you, and urine culture with you, is that pretty conclusive I had no other forms of NGU, or does that stuff avoid detection in the ways I just described (plus lack of urinary discomfort, discharge, urinary frequency).
3. I got a new coin-sized rash at high-inner thigh, near scrotum today (of course, it’s 100 degrees out) and scrotum is red on sides with white line through middle. Sound like anything? I’m going with the “it’s hot and fungal” explanation and put a fungicide on again. Other “pain” symptoms persist, especially right inguinal.

Update: MG test came back negative.
Rash on thigh near scrotum is largely flat, not really bumpy. Maybe one small bump I can’t see too well. Photo attached but it’s hard to see-see area near finger.
My explanation of scrotum earlier was poor. It’s mostly red, with no bumps. But area in middle, where my penis rests on it, is a more normal, paler flesh color (not white like froth or chalk).

Hi again, Terry-happy holiday to you.
So I have had the bright red scrotum with the line through it for some time-weeks. You saw it when I had you look for epididymetis, though I am not sure I mentioned the color issue (I should have).
The small coin-sized patch is new, and kind of alongside the scrotum. Extending from there on right thigh is area with small, raised red dots. In addition, I have noticed red skin moving back from scrotum in direction of anus.
Sound like anything you’ve seen before?
That goes with other symptoms I have had-rashes on upper thighs (behind scrotum), various pains near anus, stomach, perineum, back, back of legs, shoulder, tingly hand that falls asleep, two-month pain on right inguinal, some recent pain on left inguinal. I am three months post-exposure.
Are you familiar with red scrotum syndrome? Sounds similar to what I have (in addition to whatever else). Sounds like it’s a mystery but possible explanations are fungal, bacterial, herpetic, and guilt/anxiety, all in play for me. I began using an antifungal directly on my scrotum today in addition to on new splotch-hoping that will help. Do you have a sense of what causes RSS?
Is four months good time to take western blot to rule out herpes? Or six?
If I am one who never becomes Igg positive, is that true for herpes 1 and herpes 2, or is that unrelated?

No, it changes nothing

I think you can be clear that you don’t have NGU. Every possible test has been done that could be done.

This looks and sounds like a fungal infection to me. When you treat with the antifungal medicine, be certain to treat twice a day for at least two weeks. We are seeing so much fungi in these hot summer days.
The western blot at 16 weeks is perfect.
How about if you try to let this go, just for the weekend and celebrate the 4th of July?

Any thoughts on the red scrotum? Should I use antifungal there or will that make situation worse?
Also, if I have sex with partner, and I have fungal infection, can I give it to her?

Yes, yes antifungal on the scrotum, for sure!
Usually the fungal infection that men have is not transmitted to their female partner but the reverse happens pretty often. I can’t tell you that transmission to her is impossible but it is really unlikely and it if was transmitted to her, I would not really consider that an STD though technically it is but not in the sense that you are concerned about.you have a good holiday, too.

This is my scrotum.
I don’t think it’s quite this red in real life, but you get the idea.
Note the flesh-colored stripe in middle.
Associated with herpes, any other STD, or anything else you’ve ever heard of?
I have read it being named red scrotum syndrome.
I also wonder if it might be contact dermatitis from my wailer round of antifungal.
Some say it’s all guilt and stress.
Thoughts?

So do you feel like it is worse since you put on the antifungals? If yes, then perhaps it is a sensitivity reaction to the medication.

No, not worse since I started back on ant-fungals a day ago.
I wonder if it started shortly after I started anti-fungals seven weeks ago though. That’s when I began to feel the occasional itch at top of penis too.
But my scrotum was basically like this when you checked me for epi too.
Other ideas?

Well, whether it is a reaction to the antifungal or a fungal infection, neither is an STD.

New bump on buttock appeared yesterday.
It’s on it’s own, nothing near it, though I have some red marks in sacral area too.
Look like pimple or herpes?

How much does swab test cost?
Can you swab multiple areas and what does that do to cost?

I am beginning to wonder if I got hsv1 based on stuff I have read here about that igg test being so bad.

You did NOT get HSV 1 from that encounter. But you could have HSV 1 already and the screening test missed it, yes.
I’m not sure about the cost of the bacterial culture but the PCR for herpes is $75. I would like to swab for both bacteria and herpes IF it looks anything like herpes when I see you. I just can’t tell from the picture. Yes, you need an appointment but I just this minute had a cancellation this afternoon if you want to come in. We an also do a herpes western blot if you are still concerned about it missing HSV 1, but we won’t do it to rule out HSV 1 from the encounter we have been discussion because I do not believe that that encounter put you at any risk.Terri
