My wife and I recently got full STD panels (example for kids) for the first time since getting married 25 years ago. She was negative (<0.91) for HSV1&2. I had an HSV 1 IgG of 51.9 and HSV 2 IgG of 2.03. I got a follow-up Quest HSV 2 Inhibition (from what I’ve seen the Quest Inhibition can be as accurate as Western Blot but has never been officially evaluated on asymptomatic) with a screening index of 4.72 and % inhibition of 78.9. Neither of us of ever had any symptoms (I realize asymptomatic is common). My last possible exposure was 28 years ago.
Is is possible that I was exposed before we were married and never passed it to her? (Or more likely that one of us has false results?)
Your HSV 1 is a solid positive, but your HSV 2 is unclear. We have seen false positive inhibition assays (positive on IA but negative on western blot), so I’m not clear that I would believe that quite yet – but your call, of course. I personally would never rely upon this test as the final answer. Also, the IgG test misses 30% of HSV 1 infection so your wife may or may not be infected.
The western blot is your best answer if you want the best and final word on your HSV 2 status.
Also, if you do have HSV 2 but have not passed it to your wife in 25 years, you certainly aren’t shedding the virus very often. Same is true for your HSV 1 infection.
And yes, if you truly are positive for HSV 2, you could have been infected years ago, but I’m still skeptical of your result.

Is it true that most asymptomatic shedding is within the first few years of exposure and then drops after that?
I guess I don’t have an immediate need for confirmation via Western Blot – is there a way to contact you directly if I decide to do so in the future or would I still need to go through the eVisit process?

People who acquire herpes Shadmoor both with and without symptoms in the first few years than they do subsequently. If you are interested in obtaining a western blot with my help, you would need to go through the Evisit process, correct
