Oral HSV2, any chance of false positive?

      Hi Terri,

      20 y/o female w/o any symptoms that I know of.

      Background: Recently tested + for HSV2,& wasnt told my index value. (Yes i know thats most important, will get it soon).
      Abt 4 years ago, gave oral sex to guy I dated briefly. ONLY 1X. Dont recall if he touched himself, but did finger me. I masturbated him, gave oral for couple mins at most then masturbated him til he finished. He had sex for the 1st time with 1 girl ~2.5 months before this. No idea abt her status or history. Also know he cheated on me, but dont know details. Dont know his status, just that he had no sores I could see/feel.

      Next guy was 5 years ago. Probably a max of 5x, he masturbated himself & ejaculated into my mouth,but realistically his penis probably had some contact w/ my mouth as he finished. 1 time he begged me to put my mouth on it,& I think I did 1 time, very briefly. Encounters a handful of times w/ this guy w/ some, but not a lot of oral contact w/ his penis. Most contact I had was w/ semen, which doesn’t carry virus, right? Also fingered me a couple times, but not when he masturbated. Dont think he had other partners at the time, but cant be sure. Didnt see/feel anything abnormal, but cant say I looked hard.

      I probably was sexual w/ bf shortly (month or 2) after 1st guy. Think I was w/ my current BF during encounters with 2nd guy. Regrettably was unfaithful. So I might have been sexual w/ 2nd guy & w/ BF not too long after.

      IF I did contract herpes, big chance I already spread it to my BF. But hes had no symptoms whatsoever. I know this doesnt mean we dont have it, but it seems hard to believe that 3 people would have no symptoms at all after all these yrs.

      I plan to have my BF tested, but for now this is all I have.

      My questions:
      1. Based on 1st encounter, IF he had a recent infection, what are chances of virus being active enough to infect my mouth even with just one encounter?
      2. Based on 2nd guy, what are my chances of contracting oral herpes?
      3. I had braces during this, so while i dont know if this was true, theres a chance of little cuts in my mouth from braces. Does this affect likelihood of contracting hsv in my mouth, even being away from site of pref.?
      4. Is there any real risk that I did get infected genitally from their fingers? Never any genital-to-genital touching w/ them.
      5. If I do have it,& my partner is uninfected, how likely am I to pass it on if I am only infected orally?
      6. Does it sound possible/likely that I did contract it orally, then give it to my boyfriend on his genitals, then he give it to me on my genitals too all within a few months? Is the virus that active even w/ oral hsv2 infection, that it could spread that quickly?
      7.Is oral HSV2 ALWAYS a low shedding rate, or is it just as high as genital hsv2 in the first few wks/mos?
      8. Is it common to have no symptoms when 1st infected orally?
      9.Why is oral hsv2 so uncommon,& what exactly makes it “harder” to transmit hsv2 from genitals to mouth?

      Sorry, forgot to add I am basically positive tests were type specific IGG b/c medical assistant assured me it was type 2, not 1, and that there was no way of knowing how long I had it (leading me to believe it wasn’t IGM).

      If we were talking about HIV, I would be more concerned but not so much about herpes in this situation.

      Unlikely as HSV 2 is shed infrequently from the mouth.

      6. Does it sound possible/likely that I did contract it orally, then give it to my boyfriend on his genitals, then he give it to me on my genitals too all within a few months? Is the virus that active even w/ oral hsv2 infection, that it could spread that quickly?
      No, all of that sounds way too complicated and unlikely to me.

      7.Is oral HSV2 ALWAYS a low shedding rate, or is it just as high as genital hsv2 in the first few wks/mos?
      If you have it orally, shedding would likely be more common at first, as it is with HSV 2 genitally, though we have no hard data on this
      8. Is it common to have no symptoms when 1st infected orally?

      Yes, it happens.

      9.Why is oral hsv2 so uncommon,& what exactly makes it “harder” to transmit hsv2 from genitals to mouth?

      Because HSV 2 has what we call a site preference for the genital area. we don’t know why, it just definitely prefers the genitals to the mouth. HSV 1 is far less picky.

      Thank you, this sounds reassuring. However I am so depressed because I just noticed a big fluid filled bump on the back of my boyfriend’s thigh, I’ve never ever seen anything like that on him before. I’ll attach a picture if I can. He said it looked like an ingrown hair and hurt, so he squeezed it Thursday, then it apparently never went away and got big and ugly looking. This was Saturday night when I saw it. He said its cause he’s been picking at it, but it did not look like an ingrown hair to me. The next morning I looked at it, and it was gone. I’ll post a picture of that too. Last night it looked scabby but may have been a sort of open sore. I don’t know I’m just freaking out. I know if I have this, it happened orally, as he’s the ONLY man ive ever had genital-to-genital contact with. But the chances seem so low 🙁

      My follow up questions are:
      1. If it is an old infection, are my antibodies likely to be high, making a low positive a source of hope?
      2. Are herpes stores ever cuts that bleed? Much like an anal fissure? I get these occasionally, but it’s usually after a couple days of diarrhea or wiping a lot. I also get inflammation/little cuts on my vulva, but my dr. said its sort of like eczema and said I can use steroid cream. This usually happens during my period because I wipe a lot and use tampons. I do get frequent itching, but I can recall this problem dating way back before I was sexually active. My mom used to put Vaseline on my privates when I was younger so I think my skin down there is just very sensitive. Do these sound more like herpes symptoms, and I’m just in denial?
      3. Is it common to have a herpes sore burst and scab up hours later almost like nothing was there? This is what happened with the thing on my boyfriends leg.
      4. Is it common to have only one sore that looks like an ingrown hair, opposed to the usual clusters?
      5. Is it possible I DID give it to my boyfriend from my mouth to his genitals, shortly after I contracted it? And so maybe my possible genital symptoms really are coincidental and I only have it in my mouth and he has it genitally? I know unlikely, but do you believe it could happen?
      6. Given the pictures I attached, does this look like herpes? If so, is there any indication whether it is primary or recurrent? He says he has got ingrown hairs before but now I question if they were herpes all along. I pray you say it doesn’t look like herpes.

      Thank you so much for answering these questions. There’s so much unreliable information on the Internet and, after reading your forum, even some doctors. I have been SEVERELY depressed over this and the guilt of contacting this from a stupid guy and giving it to the man I love 🙁 I can’t forgive myself and all I do is pray this isn’t true or somehow God will heal me. (Probably sounds ridiculous to you, I know). But it’s my source of hope.

      Usually with old infection, antibody levels are traditionally higher.

      These do not sound like herpes symptoms at all to me.

      Yes, I believe that that can happen but it’s usually not hours and then there’s nothing that sounds less like herpes. However, the picture does concern me did he have this swab tested at the time that it was present? I forget– did your boyfriend have an antibody test?

      Either are possible

      Again I don’t think you have much risk and so I think it is unlikely that you gave it to him through oral sex.

      It’s very difficult for me to say whether this could be herpes or not. If it didn’t have the big red area around the lesion, I would be less concerned about herpes. Again, my recommendation is that he have this lesion swab tested for herpes and that he should get an antibody test to try to clarify the situation.

      1. If his lesion is already scabby looking would it still be beneficial to get a swab, since all the fluid is gone? Can we go to an urgent care to get tested? He gets off work late in the evening making it difficult to make any appt this week. By the weekend it will surely be totally gone. He’s already getting agitated over my obsession over the lesion 🙁

      2. Have you ever seen herpes infection because of finger transmission? This is literally the only other way I could’ve got it. Isn’t it even more rare than oral though?

      3. Based on me only giving unprotected oral sex and being fingered by each guy, *assuming* they could be infected, would you say this is low enough of a risk that I probably am not positive? Or is it likely enough that if I am positive, it had to be one of those occurrences? I just can’t wrap my mind around how something so rare happens. And if that sore is herpes, I did give it to my boyfriend from my mouth to his genitals. It’s all so unsettling.

      4. Would lower abdominal itching have anything to do with herpes? It’s like a deep itch across the bottom of my abdomen.

      5. Does spotting have to do with herpes? I’ve always had an irregular cycle so I always thought it was just because of that. I’m on the pill and it usually happens mid cycle, this time it has lasted at least 3 days. It’s not heavy and never hits my underwear, it’s just either pink or brownish when I wipe. Could this just be due to how stressed I am?

      6. Could the redness have to do with him picking at and irritating the area? I have had bad ingrown hairs that actually swelled up and felt hot, with a red circle around it. IF it wasn’t so red, what do you think this lesion could be besides herpes?

      7. Based on all of this information, would you advise we stop having sex until we get conclusive answers? He says he’s okay with it that I have it, and being with me for years, he probably already does too if I do. He’s honestly so much more calm and okay with the situation than I am – because of the immense guilt I feel.

      Sorry just to clarify, the question I asked about the redness – does the actual bubble of fluid and bloodiness look characteristic of herpes? Have you ever actually seen a lesion look like this before?

      It may be too late for testing to be done from this lesion. How about if you just back off and be certain that he obtains a type specific igG herpes antibody test.

      If I had to bet money, I would bet that you did not get herpes from either of the partners that you describe. However it is possible that your boyfriend has herpes and it has nothing to do with you.

      No.

      Spotting has nothing to do with herpes.

      If the lesion was less red, I would be less concerned. It’s pretty red to just be from picking. However it is possible and I can’t say for sure and won’t venture anymore of a guess at this time.

      I think it would be best if you stop having sex until he gets an antibody test, yes. If anyone has HSV 2 it is far more likely him then you, Given the sexual experiences you have described for me. I don’t know anything about his sexual history so I guess I shouldn’t say that but I just know that your history is not risky for HSV 2

      Hi Terri,
      Purchased some follow up questions. I’m so very confused by my situation and my anxiety has gotten bad again where i can’t sleep or eat, and my heart is beating quickly a lot of the time. I can’t quite find the answers I need on the internet, and I come across so much inaccurate info, so I decided to come back to you. This is long, but I’m freaking out so PLEASE take the time to read this to give me the best answers possible, as you always do. Thank you in advance and I’m sorry for being annoying.

      I believe you can see the original post, but I’d like to recap and add a couple things.

      I told you about two guys I had encounters with, and I was age 15 at the time of all of those encounters.

      One of the guys was unfortunately cheating. Looking at old journals I kept track of dates in, i can determine that this “relationship” – for lack of a better word – lasted for no longer than 4 months. So I can’t remember how quickly it progressed, but that 4 months was from the first time we kissed up until I moved out of that house. One out of those 4 months, he was in another state, so any sexual contact I had was with my boyfriend during that time. Then when this guy returned, I can’t recall if we did anything else, but i would guess yes, given that I know he masturbated himself and ejaculated into my mouth at least 4-5 times, which is all I can recall. I suppose I can’t say FOR SURE if there were any additional times, but it wasn’t something that happened every day or anything. So because I’m doubting myself, can we revise and make it a sort of “what if” it was more like around 10-12 times? Of these times, I stated if i actually put my mouth on his penis, it was brief. I was very uncomfortable with it and did not like doing it to anyone other than my boyfriend, but he basically begged me on occasion. So i believe i briefly gave him oral, but not deep, a couple of times probably. And I can’t quite remember, but I probably had my mouth somewhat on his penis sometimes when he finished in my mouth, and i always swallowed. There were also occasions where he fingered me. He was a smoker so his hands probably weren’t the cleanest, and I didn’t enjoy him fingering me, so I don’t remember that happening often, as in probably less than 5 times. He was selfish anyway and just wanted to get off himself.
      So to summarize, I cheated on my boyfriend with this guy for a few months, and I can’t recall any specific sexual acts my boyfriend and I did, but I know that my boyfriend did not eat me out for the first time until a few weeks-a month after it stopped with this guy. But my boyfriend and I probably did things like give him oral sex, masturbate each other, and “dry hump” without clothes while this was going on 🙁 But no sex yet.

      The other encounter was about 6-8 months after the first guy, and 4-5 months after my boyfriend and I broke up, which was me giving oral sex ONE time, probably for 5 minutes or less. But let’s just say for the sake of it, maybe I’m underestimating. So a max of 10 min. He then finished by me masturbating him, so he did not finish in my mouth. He fingered me prior to me giving him oral.

      About a month or so after this probably, I got back together with the man I’m with now and have been faithful to since we got back together. We only started having intercourse since late December of 2013, and used condoms nearly always until about 6-7 months ago, but the past few weeks I’ve been asking him to use condoms again just because i’m scared.

      Those were not the ONLY guys I’ve had contact with, but they’re the only potentially high risk people. Others were all before 15, and most only included masturbation without any oral-genital/genital-genital contact. One guy, I masturbated and put my mouth on his penis for him to ejaculate, but to my knowledge, he was a virgin. Any other contact was with my boyfriend, and he had never been with anyone else. I include these because I want to make sure that I am covering all bases and possibilities here.

      My boyfriend still has not been tested. mostly because my anxiety is getting terrible and he was getting irritated with me over the obsession. It will happen, but so far he has had that one blister, along with what I think could have been another one a couple weeks ago. I was too scared to look at it and convinced myself it was an ingrown hair.

      I also want to go over any potential symptoms I have had. I thought I never had any, but with this test, I’m now attributing everything to herpes. I can’t decipher whether I am scaring myself or if these are actually symptoms. I mentioned some of these to you previously, but I am going to again just to make sure I cover it all.
      -Always had an issue with vaginal itching since a child, but sometimes it’s excessive and very uncomfortable. Recently, I had a bad vaginal itch, followed a week or so later by:
      -what I’m wondering if it was a “tingling” feeling on my upper inner thighs where my thighs rub together and on my underwear/jeans. I always just thought it was that. but then:
      -pimple looking red bump in one of the spots that “tingled” that started to get a white head and irritation from rubbing on my pants. It is about an inch below where the crease is from labia to thigh. i put a bandaid on it to protect it, examined it yesterday, and saw a white, pus filled top to it that looked large for a white head. I had an even larger one in the past, and what comes out is a somewhat thick, yellow/white fluid. It bled a little. I thought it was a pimple, but given my anxiety, I’ve been terrified it’s a herpes sore. They’re very painful and irritating, which is why I had a bandaid on it. when I looked at it, i saw it was leaking out a bit onto the bandaid. Thought that would be normal due to it being rubbed against the bandaid and my pants. the base looked like a pimple to me and responded sort of like a pimple when I squeezed it, but it seemed as if all of the pus had been brought up to the top – not much “underneath” to bring out. so I got scared. i haven’t looked at it since then, it has a bandaid over it, and i put some bactroban on it. From what people describe, it didn’t seem like the pus was anything different than what comes out of my face pimples, and wasn’t clear, runny, etc like i thought herpes was. the only thing missing was some sort of hard center, but the base of the bump felt sort of hard. I don’t know, Terri, so confused.
      -Also occasionally have redness, inflammation, and little cuts on my vulva. There have been occasions where I have found little dots or lines of red blood when I wipe. My doctor said to use a steroid cream, which I haven’t tried yet, and said it was sort of like eczema. You had told me this did not sound like herpes symptoms, but I have been reading on sites like medhelp, and have read about women who have this as their only symptom, and they test positive for HSV2, and the irritation seemed to respond to acyclovir for one woman.
      -Anal fissures also happen sometimes, i’m not sure if that’s due to excessive wiping or dryness when I go #2. But i have also read on your forum about a woman who had these, and you said it may very well be how herpes presents for her. I also can’t remember this really being an issue since I had anal sex with my boyfriend.
      -I dont recall having any of these symptoms up until the last year or so. I had my first yeast infection/bad itching junior year of high school, but didn’t treat it for a while. 2nd yeast infection around march? 2013, and took fluconazole (SP?). Cleared it up. That was my first ever gyno visit and pelvic exam. I never had the problems with discomfort and as many bumps on my thighs as i do now. but i have also gained weight and my thighs always rub together and there’s a lot of moisture throughout these hot days.

      My questions:

      1. I read about a girl who was 18 years old on your forum a while back, and you had mentioned that based on her age, it was unlikely that she had acquired HSV2 because statistically, not many that age had it. Being that I was 15 at the time, does that have any effect on my chances of acquiring HSV2? I’m holding onto the possibility of a false positive, but I’m starting to get discouraged.

      2. Based on my “revised” sexual history, do you still believe my history is not risky for HSV2? I still have not had genital-genital contact with anyone other than my boyfriend, but the oral scares me.

      3. You said there wasn’t risk with finger transmission, but what about if someone had herpes whitlow? Is this a possibility for me getting it on my genitals? Or does whitlow present so obviously that I wouldn’t have let anyone touch me who had it? I can’t even recall what their hands looked like, but if it was bad, it seems like something I should’ve noticed. Am I at risk of getting it this way?

      4. Does herpes present as cuts on the vulva and anal fissures?

      5. As for the anal fissures, Could it be that i damaged something from the sex and have become prone to anal fissures? Or is that indication that he transferred herpes to my anus? The anal didn’t happen until the last 8 months, so I would have been way past incubation period if this is old. Or is it all just coincidental and might not mean anything either way?

      6. Does the bump/blister I described sound classic of herpes? Also, how big is a single blister typically? I keep reading “small,” but what does that mean, and can they become large, as in a little smaller than the size of a dime for the actual raised blister itself?

      7. There seems to be an alternate answer to all of these symptoms, but all together, does this sound like herpes genitally to you?

      8. If it does, do you believe I got it from one of the guys’ fingers?

      9. I asked this before, but I could have Contracted it orally, giving it to my boyfriend genitally, then him giving it to me genitally (or) My boyfriend lying about his sexual history and possible cheating on me and giving me it genitally.
      Could the first happen, theoretically? And, you probably believe the 2nd one is most likely, righT? I just have no reason not to believe my boyfriend.

      10. Assuming my boyfriend is telling the truth about no other sexual partners, would you believe that I have this genitally? Or does orally seem like the only option based on my sexual history and his telling the truth?

      11. Finally, since I have cuts on my anus and vagina, but possible little white blisters on my legs, is it possible that a genital infection presents 2-3 different ways in that area on my body? the cuts never look like sores.

      Lastly, I noticed you disabled the picture feature on here. I’m so very disappointed to learn that. I thought it was great that you allowed people to send in pictures when they were freaking out, just so they could get an idea of whether it’s something to definitely get checked out, or if it is uncharacteristic of herpes altogether. I didn’t think you were practicing medicine online, just offering advice before people got into a doctor. Is there any chance it will be enabled again in the future?

      No, there is no chance the picture feature will be enabled again. The legal advice I was given will hold about this topic.

      Yes the fact that you were 15 is even more suggestive that this is not herpes.

      I still believe you are at minimal risk for HSV 2, yes.

      Yes, it is possible that someone could have whitlow and transmit it to your genitals, but I think that is extremely unlikely as whitlow is painful and I can’t imagine anyone thinking it would be OK to put that finger into someone’s vagina.

      It could present in that but other things can present that way as well.

      5. As for the anal fissures, Could it be that i damaged something from the sex and have become prone to anal fissures? Or is that indication that he transferred herpes to my anus? The anal didn’t happen until the last 8 months, so I would have been way past incubation period if this is old. Or is it all just coincidental and might not mean anything either way?

      Anal fissures can happen from challenging anal sex, yes. And they behave very differently than anal herpes, being present a lot more consistently than herpes

      6. Does the bump/blister I described sound classic of herpes? Also, how big is a single blister typically? I keep reading “small,” but what does that mean, and can they become large, as in a little smaller than the size of a dime for the actual raised blister itself?

      Herpes lesions could come together and make a sore as big a dime but not usually.

      7. There seems to be an alternate answer to all of these symptoms, but all together, does this sound like herpes genitally to you?

      It does not sound like herpes to me.

      8. If it does, do you believe I got it from one of the guys’ fingers?
      Very unlikely

      9. I asked this before, but I could have Contracted it orally, giving it to my boyfriend genitally, then him giving it to me genitally (or) My boyfriend lying about his sexual history and possible cheating on me and giving me it genitally.
      Could the first happen, theoretically? And, you probably believe the 2nd one is most likely, righT? I just have no reason not to believe my boyfriend.

      That scenario seems extremely unlikely to me.

      10. Assuming my boyfriend is telling the truth about no other sexual partners, would you believe that I have this genitally? Or does orally seem like the only option based on my sexual history and his telling the truth?

      no, I still don’t think this is herpes

      11. Finally, since I have cuts on my anus and vagina, but possible little white blisters on my legs, is it possible that a genital infection presents 2-3 different ways in that area on my body? the cuts never look like sores.

      No, I don’t think this is herpes.

      You are greatly worried about this but what have you DONE about it? We need an index value to assess risk. We need to know if it was IgM or IgG. You need a western blot for confirmation. I don’t mean to be harsh, but what aren’t you spending as much time acting on clearing up this issue as you are typing me about it and worrying about it?

      I am also curious about how you got around the word limit on posts – this is clearly over the limit!
