Hello,
About a year and a half ago I got a big pimple in the upper middle of my penis shaft. The pimple behaved a lot like the kind I get on my face and body, with thick white pus coming out when I popped it. Also painless. I was told by multiple doctors that it visually didn’t look like herpes, and after 7 months I had negative blood test results for both Western Blot and IGG (both hsv1 and hsv2) After my western blot I went happily along with life assuming it was either just a pimple or an inflamed Fordyce spot (my genitals are completely covered with Fordyce spots)
After going almost 2 years without another Genital pimple, I discovered a white head on the head of my penis this time. I popped it and again and thick white pimple pus came out like it would for any other pimple. I showed it to my doctor and she said it “looked nothing like herpes” and was just a pimple. She said she couldn’t culture it because I already popped the white head and had no discharge. After some research, this doesn’t sit well with me because while it looked and acted exactly like a zit on my face, I’m now aware that we don’t have hair follicles on our penis glans and I’m wondering how I could have possibly got a pimple there?
I’m aware that if I get another pimple I shouldn’t pop it and jut get it cultured. But being a herpes expert, considering everything I’ve told you does this sound at all like my general doctors could be wrong and I actually could have herpes? The negative western blot made me feel completely herpes free but after having a pimple on my penis head (which I’m told is very rare) I just don’t know what to think.
P.S I’ve had 2 sexual partners. One confirmed she’s had cold sores before.

Herpes lesions just don’t look like that – the fluid in them is clear, not pus.
I cannot tell you why you might be getting pimples on the head of the penis but I have seen them before.
This does not sound herpetic to meTerri

Thanks so much for your response Terri. I have a few follow up questions.
1. How much trust do you personally put in the WB? Do you trust it to the point where you would believe a negative WB over unusual symptoms on the genitals?
2. Am I misunderstanding “atypical” symptoms? As in, are herpes blisters and oil/pus filled pimples always caused by two different things?
3. My pimple/Whitehead pretty much stuck around unchanged for about a week before I popped it, then it healed in about 2-3 days. Would a herpes blister typically pop long before a week of no changes?
4. Are sore tonsils a typical symptom of a reoccurring outbreak? I had sore tonsils at the time but my throat acts up a lot. My doc seemed to think it was unrelated and allergies.
5. Finally, I seemed to have a slight burn/sensation on my scrotum at the time. To be honest I’m not ever sure if this was just in my mind or not. The sensation was on and off throughout the week. Would Herpes efefct my scrotum even if it’s not the location of the lesion?

yes. I do not trust it over people who have a positive swab test for HSV from a lesion in the genitals

Atypical would be like cracks, splits, things like that

No, not normally

Based on what you’ve told me, if I get another Genital zit as described do you recommend visiting a dermatologist and searching for other causes than herpes and end the HSV testing? Thanks for all your help.

Yes, that’s exactly what I would recommend. Be sure to get in right at the beginning of any genital lesions, including what you think are likely zits.

Hi again, unfortunately I’ve purchased new questions. I’ve now spend hundreds and hundreds of dollars on the herpes which blood tests tell me I don’t have.
I just visited my dermatologist. She examined what was left on the white head on my penis (it’s formed into an extremely tiny bump that doesn’t change in appearance). Like everyone else, she’s told me that she doesn’t think it’s herpes.
Questions:
1. I’ve had another sexual partner in between my first and second penis pimple. This week I took an IGG test that came clean for both hsv 1 and 2. Would it be a waste of money at this point to get another western blot?
2. When you say herpes blisters don’t have thick pimple pus, do you mean literally never? Like in no scenario at you’re clinic you’ve seen herpes present as a classic pus filled pimple?
3. My first pimple appeared into a big Whitehead within literally a few hours. After seeing nothing on my penis hours before, I noticed it while using the bathroom and was really big. Is this concerning in terms of herpes compared to a pimple?
4. Is a negative western blot conclusive enough to not disclose and it would be ethical?

1. Do you know the antibody status of The newest sexual partner? If not, you may wish to ask them to be tested. Again, herpes does not form a big Whitehead within a few hours. A negative Western blot is certainly conclusive and you do not need to disclose that you have herpes when the best test possible says that you do not.

Thanks for the reply. As bad as it sounds, I’m not in contact with my last partner as it was overseas on vacation. I will take your advice on the non disclosure because of my negative WB. 1. If you were in my position would you pay to have another western blot done? 2. And a random question; If I were to get Hsv1 on both my mouth and genitals would I see initial symptoms at the same time?

No, I would not redo the Western blot. if you did acquire HSV one both orally and genitally at the same time, you may or may not have recurrences at the same time.

Can you explain to me the process of having a video consulatation with you in order to do the western blot including an estimated total price? I’ve ordered one because my second pimple occurred in the same time frame as my latest partner. I live in the south and I literally can’t find a doc who will work with third party testing.
