Hi Terri – Please help me understand where do I stand in this mess. I’m too stressed.
Again, thanks for your time. Here is the timeline for the events:
25 Saturday July 11:45 PM – deep open mouth tongue kissing for a minute. And then, for less than a minute the girl performed oral on my penis (it was short and brief). Then, I suck her breast nipples (very quick) for less than a minute.
27 Monday August – All symptoms inside my muouth. I felt burning in my bottom lip inside my mouth. I was not able to eat for next 4 days (very painful). My bottom lip was red – felt like cold sores(wet- kinda like – canker sore). No medication taken for 4 days.
28 Friday August – I go see a nurse practitioner. She gave me 4 tablets of valacylovir and I felt good after 3 days and cold sore were gone after eating valacylovir 1000mg.
On 4 August – On 10th day after exposure, I ordered a Std test. HIV 4th generation 1/2 antigen/antibody with reflexes, hep b/c, hsv2, chlymadia, gonorrhea, all came back negative. Only HSV1 came positive >5.
On 7th August – During all these cold sores, my right eye was red. I go see an optometrist and he gave me viral eye drops and I started taking 3 tabs acyclovir a day for 7 days because under my tongue a new cold sore/- no improvements in mouth. But, no pain.
On 11th August – I go see a MD eye doctor. According to her, I have conjunctivitis/pink eye. I stayed on Acyclovir and started putting ofloxacin eye drops. Eye is doing very well now.
On the 13 August – I ordered a HIV 1 RNA test after 18 days of exposure and It came back negative.
On 14 August – I did go see a General Physician doctor and she said, it looks like canker sore. No change in medicine.
On 20th August – I go see my reglar GP because my sores/canker sore are not getting better at all. I’m back on valacylovir from last 9 days and 1 more days I have to take valacylovir – it’s working good. According to doctor, I was diagnosed with Herpetic gingivostomatitis. For next, 30 days I will be on 1 tab of Valacylovir 1gm/day.
On 28th August – I go see my reglar GP, she offered if I want to see Infectious disease specilaist. I said, I will take this medicine and see how it goes.
It has been a 35 days now and I’m still having cold sores/Herpetic gingivostomatitis(not sure what is) inside my mouth (due to antiviral intake – severrity is less). Yes, sores are clearing up after valacylovir. But, again new ones keep coming. These sores start 1mm in size and might go over 1 inch – this is the one located at the outside right bottom gum. From last month, oral hygiene/eating habits/exercising was not that good. So, I saw tongue colored bumps on side of my tongue and tongue started to turn white. My GP gave me an oral solution to rinse with for next 10 days. So far, it has not increased, but stayed as is.
Questions:
1. Terri, so far, according to nurse practitioner and GP, they are calling it cold sores, canker sore, Herpetic Gigantistomatis etc everything inside my mouth/cheeks/gums. I’m really worried about what exactly it is? Will I be ok soon? Please help.2. Big one – HSV2 Possibility – Based on incident, what is the probabilty that its an Oral HSV2? So far, all symptoms inside mouth, no genital symptoms. I’m too much stressed as I’m trying to dig deeper.
3. Lets assume the girl have hsv2 oral – what are my chances of acquiring it from her in this kissing and oral performed on me?
4. Test Results – I know tests were conducted too early as I gave blood on 10th day after exposure, HSV1 IGG = positive; HSV2 IGG = positive. Still test was able to pick up HSV1, I am assuming if I had hsv2, the test would have picked up. Please comment.
5. Do you see any other possibilities of STDs which I have caught from this incident?
6. Seems like I would need a phone consultation with you. Please let me know what are the next steps for phone consultation.

I have no idea what it really is but has anyone done a swab test either for culture or PCR? You have a well established HSV 1 infection with an index vaue at >5. You did not just get HSV 1. Cold sores and canker sores are completely different and I’m not sure how anyone can call this herpetic without a swab test.
2. Big one – HSV2 Possibility – Based on incident, what is the probabilty that its an Oral HSV2? So far, all symptoms inside mouth, no genital symptoms. I’m too much stressed as I’m trying to dig deeper.
I would say it is as close to zero as it can be without being zero.

Why are you saying the HSV 2 was positive? I thought only the HSV 1 was positive? I’m confused. If you did acquire HSV 2 orally, which I would be a million dollars you did not, it would not show up 10 days later, no

I think there are other things it could be like other viruses or bacteria. Has any testing actually been done other than a blood test for HSV antibody?

I am out of the country for three weeks and won’t be doing phone consultations in the near future. But I would ask your health care providers to consider swabbing for other viruses or bacteria. Herpes outbreaks also don’t last a month. Where you live?

Thanks for your prompt reply. Terri, I have say that, you have a great site/forum/books and it provided me with excellent information. And, I do want to keep you in the loop so that you can guide me in right direction.

You have a well established HSV 1 infection with an index value at >5.You did not just get HSV 1. – I know its tough for you to say, but what else you think? May be another bacteria/virus as you mentioned earlier?
Cold sores and canker sores are completely different and I’m not sure how anyone can call this herpetic without a swab test. – AgreeNurse Practionner – prescribed medicine based on my visible symptoms on the very first day. Then, on 10th after exposure, I ordered STD panel test from Quest diagnostics, results:
HSV 1 IGG TYPE SPECIFIC >5 = came positive
HSV 2 IGG TYPE SPECIFIC <0.90 = came negative
hep a/b/c; HIV -1 RNA (95% accuracy after 2 weeks of exposure), chlamydia, gonorrhea all came back negative.

I think there are other things it could be like other viruses or bacteria. – Agree, I had a discussion with HIV/AIDS specialist, according to him, due to kissing and intimacy other symptoms are visible which are not related to STD infections.

Where you live? – IllinoisQuestions:
A. Lets say, after 6 weeks, I order another blood test IGG FOR HSV (I know you consider blood tests after 12-16 weeks) while I am on Valcylovir one a day, will this antiviral a day affect my HSV test results?B. My general physician have offered me if I want to see Infectious disease specialist. In your opinion, should I go see specialist?
C. As I’m too much stressed 24/7. Can these symptoms due to stress?

Yes, the antiviral therapy can impact the test results, definitely.
If your doctor doesn’t know why you have ongoing symptoms in your mouth perhaps a referral is in order. But I honestly think the chances that you acquired HSV 2 orally is very very small. Let’s say worse case scenario she has HSV 2 orally (rare). Then she would only shed virus from her mouth about days out of the year – what is the chance it would happen on that particular day?
If you have an actual sore in your mouth I doubt it is actually due to stress alone.terri

Hi Terri – please help.
In September, I did consult ID specialist, he ran CBC and other tests, all came back normal. He took me off Valtrex while my inside mouth is full of sores and he said, howcome on Valtrex, I’m getting these sores? According to ID specialist, it’s something else in my mouth as my body is reacting to something. My mouth clear off in 10 days. Basically, I was off Valtrex for oct and November – my mouth kept on getting sores under my tomgue, tip of tongue, right on top of wisdom tooth, inside cheeks etc.
Nov 28, first time ever I feel burning on top of my lower lip. I stop by to see GP – she diagnosed me with cold sore – at the same time inside of my mouth was full of sores all over. On December 4, I’m back on Valtrex, lower lip sore goes away in a week. Since dec 4,I’m on 1gm Valtrex a day; my inside of mouth keep on getting sores over and over and then they heal.
1. Is it really herpes inside my mouth? while I’m on 1gm Valtrex a day?
2. Can someone keep on getting sores while someone is on antiviral? Below tongue, tip of tongue, red pallet, top of wisdom tooth etc
3. What if I do a biopsy of mouth sores as they keep on coming and heeling? Will it show hsv types? Hsv1 or 2?
4. Back on Valtrex dev 4, My first oral OB back in July 25; till dec 15 nothing around genitals area. During this time of 4-5 months, I was on and off Valtrex. On December 15, I see 3 skin colored bumps on my scrotrum. I did stretch my scotrum’s skin, there was very minor itch, I took shower and no burning, no pain. I did apply a anti itch cream and in a week or so I don’t see any bumps. I don’t believe these are genital HSV symptoms? Am I correct?
5. Request – I have been dealing with this mess for a while. Terri, can you please let me call you for phone consultation? Plz? I would really appreciate your help. -

I would have to say that herpes rarely occurs inside the mouth and I would agree with your ID doc. It sounds very much to me that this is being caused by something else. What did the doc think was causing it? And herpes doesn’t keep staying around like this. Did someone recommend a biopsy? The genital symptoms do not sound like herpes to me, no. While you are infected with HSV 1, these symptoms don’t sound anything like HSV 1 to me. Has anyone swabbed the lesions for HSV? I mean, that would be the most normal thing to do, IMHO.
We are no longer allowed to do phone consultations just to talk about herpes but we can do phone calls to talk about participation in the research study utilizing the western blot, so if you at all wanted to do that, we could talk about it, for sure. But I doubt that I have anything to offer you on the phone that we can’t talk about right here.

Hi Terri;
Since dec 4, I am on Valtrex 1gm daily. ID specialist swabbed by mouth full of sores and results came back negative for type 1 and type 2.
1. Random question. While on daily Valtrex therapy – are these swab results conclusive for HSV?
2. Based on our discussion, what are my chances that I might have HSV genital?
3. Can genital symptoms appear after 5-6 months?
4. If someone has gental HSV, are they suspected to have other STDs?Again, thanks for your time and help.

1. Swabs gathered while on herpes medication may not be reliable.
2. I doubt that you have genital HSV
3. genital symptoms can appear months after infection, yes.
4. If someone has newly acquired HSV , we do like to look for other STD’s yes.
5. Your numbers look like you have HSV 1 but not HSV 2, at this point.Terri
