Hello Terri,
I am a 19 year old male, and I had my first sexual encounter on the 28th, 9 days ago. No penetration, just receiving oral sex on my penis. I never made any contact near my anal area.
This morning, I woke up with a lot of itching on my anus, and I went to check and take pictures of it and noticed two bumps – one above the anal cavity and one below. These are abnormal, but there was no pain or tingling, just itching. I went to get tested today and the nurse practitioner told me she’d be surprised if it was herpes, but that she was unsure. She swabbed the possible lesions and said I’ll get my results back Friday or Monday. She said that the one below my anal cavity looked like it was possibly ulcerated, but said that it was unlikely to be herpes if my anal cavity never made contact with a sore or saliva and that I lacked traditional herpes symptoms.
I have no history of cold sores and therefore no immunity to HSV1. My sexual partner told me he has never had cold sores either but I understand that people often times do not know that they have HSV1 and can still spread it without an active outbreak.
My questions are:
1) How likely is it that somebody without an active outbreak who just gives oral sex to a penis can cause a primary herpes outbreak near my anus with which his mouth never made contact? Have you ever encountered a case where somebody had anal herpes without ever having actual contact with a sore?
2) How often are primary GHSV1 outbreaks in a location different from where contact was made?
3) Can I expect the swab test to be accurate?
4) Is there anything I need to look out for to make sure this isn’t an outbreak? Is there anything I can do in the interim to reduce/stop it if it is one?
5) Could this possibly be HSV2?
Thank you for reading and thank you for your time. I am so confused and I feel so alone. This is my first sexual encounter and I feel so ashamed and hurt by this situation and I don’t know what to do looking forward.

I think it is highly unlikely. Normally, people get a first time outbreak in the location where they sexual contact occurred – your penis.2) How often are primary GHSV1 outbreaks in a location different from where contact was made?
Very rarely3) Can I expect the swab test to be accurate?
Yes, if it was a PCR or NAAT test4) Is there anything I need to look out for to make sure this isn’t an outbreak? Is there anything I can do in the interim to reduce/stop it if it is one?
I don’t think anything would be helpful at this point5) Could this possibly be HSV2?
I would say that’s a one in a million or more possibility – that your partner would have oral HSV 2 and transmit it to you.
