Hi,
I am in a committed relationship that has been tough. I had a vulnerable moment (1 time) new encounter that was a mistake. We used protection of course, but oral was not. So 4 days later, I went to a testing lab, and had a full panel done. Everything was negative including hsv1. The Hsv2 igg test was index of 21.9. I was floored. I didn’t have any symptoms at all. So a day afterwards, I was really stressed and started feeling needles all over my body for about 3 weeks. I had a follow up apt with my PD for something else and showed them the test. They told me not to worry that I was fine, and as long as I didn’t have bumps for sores.
17 days after encounter, I noticed burning after unrination during cycle. At 24 days after NE, I went to my gyno, she said the same thing as my PD. She swabbed the area and discharge. Both were negative. No yeast. Took full blood panel again, Only IGG Hsv 2 18.7.
I asked about meds they said I didn’t need it b/c I didn’t have a true visual case. Some cream was prescribed and it did calms things down, and V. I notice a tiny red bump on my top thigh and then below my belly button, no clear liquid or puss, they don’t bother me but they have been there for weeks. I developed a rash at the top of my bum cheeks, PD gave a cream and it’s much better but seems to burn some. 4 days ago I went to a different gyno, b/c I noticed tiny red area where I urinate, burning was more pronounced during cycle. Took V. Seem to feel better. At gyno visit she stated things looked normal on outside but swabbed redness, and saw a lesion on cervix. Swabbed discharge, and bloodwork with Igm. Waiting for tests. I haven’t had any bumps, sores, just burning after urination and white discharge no odor. I’m not sure if this is my PO, or something else, I’m so confused.My question is it possible for antibodies to show up 4 days after new exposure with a high index and what does that mean? Or, would this result be from a previous/older exposure.

I don’t know what brand of test you had. It would help me enormously to know what lab did your test so I could see the “normals”. Do you happen to have those? If it was one of the usual labs, LabCorp or Herpeselect ( I doubt it was that), then you have a very well developed antibody response. That did NOT happen in four days. Again, depending upon the test that you had done, you’ve likely had herpes for a while. Which, in your situation, is likely a good thing. Has your regular partner been tested?

Thank you for your quick response. The first test was local testing place who used Labcorp. I’m not sure which type my 1st gyno used, I can ask. I do know it was igg.
No, I don’t believe my long term partner has. I’ve never been tested either. Do you think this could be from long term partner? -

Yes I do think that, or someone even before this long term partner. Your long term partner needs testing, either way, don’t you think?

Yes, absolutely. I have one more question. In your professional opinion, do you think with the symptoms I stated earlier, is this my PO? Or could it be something else. The burning after urination has lasted about 1 month and 1 week. The discharge (wht) almost a month.

No, I don’t think you noticed your primary outbreak. Or you attributed it to something else. If your LabCorp value was 21 four days after the encounter, you didn’t get it from him. Period. Symptoms that last a full month likely are not caused by genital herpes, especially an old infection like I believe you have.
You are absolutely the perfect example of why people need baseline antibody testing after they have a risky exposure. If you had waited to test until 16 weeks later, you could never have known if you had herpes from that encounter or had it previously.
