Hello,
This forum and site have been a great source of information. I am dating a girl who is HSV2 positive genitally. She has had it for years, and is on suppressive therapy. I have never been tested for the virus before. I have never had cold sores orally that I can remember. We discussed the risks beforehand and agreed we would not engage in anything if she was having an outbreak. We had our first sexual episode 19 days ago. To be specific, we were both naked, and I used my hand/finger to pleasure her in/on her vagina. Afterwards, she performed oral on me unprotected, and finished me to orgasm with her hand. No protection was used in either case. I really didn’t think we did anything that would have put me at risk for getting the HSV2 from her. The only possibility that comes to mind is using my finger inside her and then accidentally touching my penis or thigh’ About 3 days afterward I woke up and noticed a small blister-like sore on my inner thigh, halfway between my knee and groin area. It seemed to be a little itchy, and looked like I broke it open because it was probably bothering me during sleep. It seemed to be dry and heal really quickly, and just looked like a little broken skin after a day or 2. It appears to have a leg hair right in the middle of it. At this point 15 days later it just looks like a barely visible red mark. At day 4 or 5 after sex, I noticed at the gym that I felt rundown and didn’t have nearly the energy I usually do during workouts. The other symptoms that I have noticed that are ongoing are a little pain from my penis. It feels like there is a low grade pain just inside the tip. It seems to come and go. I have also had tingling sensations that I feel all up and down both thighs from the tops down to my knees. It kind of comes and goes. Sometimes I feel it most before bed, but its not there in the morning. The best way I can describe it is not pain, but the tingling that almost feels like if you put clothes on out of the dryer that have a lot of static built up in them. Sorry, I am trying to be as descriptive as possible! No other rashes or lesions to speak of. How much risk were the things I did with her, what should my next steps be if these symptoms continue? Does this sound like I have gotten HSV from my partner? I have a doctors appointment with a general practitioner/internal medicine doctor on Monday. Would I be better off going to a clinic/planned parenthood type place instead? I’m not sure if my insurance would pay at planned parenthood. What tests should I have him do ? From reading, I guess a herpes test is not going to be conclusive after such a short time. I thought I would have him do a full STD panel to get some more info. Sorry this is so long. Wanted to get down all the info I could think of. Any insight and information is very much appreciated and thanks again for this service! -

You did not do anything that would put you at risk of acquiring genital herpes from her. You were very cautious.
I think what you need to find out is if you have either HSV 1 or HSV 2 already. The test that you want is an IgG antibody test – under no circumstances should you obtain an IgM test. They have many false positives and make people fearful when it is not necessary.
The symptoms that you are describing are very likely due to you over observing your body for signs that you have acquired genital herpes. If you go farther with her sexually, my guess is that you will continue to notice every little thing in that part of your body, which normally would not have been noticed by you, much less a concern. This is a pretty typical reaction for someone having new sex with someone with herpes.
Once you get your antibody test back, you will know more about how much you need to worry, if at all. You can also order an IgG antibody test for HSV 1 and 2 online at many testing sites if you would prefer to go that route, might be easier in the long run but they often don’t take insurance for payment.You have two more questions so let me know if there are other things that come to mind.

Hello,
Wanted to ask a follow up question. I went to my internal medicine doctor and had him screen me for all STD’s Everything came back negative. Unfortunately he did an IGM HSV test instead of IGG. The test was specifically a HSV 1/2 IgM Ab w/Rflx Titer. It came back negative for HSV1 and negative for HSV2. The test was done at day 23 after my possible exposure. Can I take any relief knowing the result of this test. Should I still go back and get an IgG test done? Still feeling tingly in my thighs and groin and stomach area. Much less after hearing this negative result. Thank you again for your insight! -

Just wanted to add he did a full physical on me and checked my groin area. I showed him what is left of the sore on my thigh. He said it didn’t look like anything and not to worry about it. He also didn’t seem to think much of my symptoms, and that I should not be concerned.

Unfortunately, the IgM test is not reliable and I have no idea why your doctor would order that test. What you need is an IgG antibody test to determine if you have been infected at some point in the past. It may not accurately reflect an exposure 23 days ago, however.

It has now been almost 4 weeks since the past exposure. Do you think I should take the IgG test now to get a baseline on my status, or should I wait until 6 weeks or maybe even longer to? Does the IgM test tell us anything at this point? I understand if anything it is sensitive and gives more false positives. Does it make it lean towards less likely that I was infected in this last event? Thank you for your time, please let me know if I need to add for any additional questions.

I believe that I already told you that you were not at risk at the sexual experience. I think you need establish a baseline value, but I see no reason to test for the experience that you had with her. But I do think you should find out if you have herpes from some previous sexual experience in your life, just to be sure! The IgM test is useless. It doesn’t make me think anything in particular about your situation. It just makes me think that physicians should learn that IgM testing for herpes is not at all useful. I believe that you are not at risk of acquiring herpes in this situation.
