      I started dating my partner and we never had sex of any kind until monogamous. The first time we had any kind of sex was around 12/2021. I have had very few partners and never been positive (confirmed) for anything. I hadn’t had a partner in several years prior to him and no sexual activity. This month on 2/15/23 he had a fever blister pop up. I had no idea he had a history of those or that they could be spread to genitals. We had sex (oral and vaginal) on 2/14/23. Around a week later, I noticed I had painful urination and assumed it was a UTI. I was prescribed an antibiotic, treated for a yeast infection, and took azo (this helped the burning). I inspected my vagina and noticed one small, painless to touch, clear, fluid filled vesicle. Aside from this, I continued to have painful urination. This spot went away with no crusting or scabbing. I had two subsequent red bumps on my buttocks pop up in the bend of my leg. These were relatively painless. They have scabbed and seem to be disappearing. I was not able to get them swabbed, but took an Igg test showing no exposure to HSV 1 or 2. I have asked my boyfriend to repeat his test (he’s now shared his old one and it showed HSV 1). We don’t have the results yet. I’m terrified and have been in a state of depression, as I greatly prioritize my health and the health of others. It’s hard to tell what my life will be like. I have two days left in my Valtrex course (I ordered online and started on 2/21. I don’t know what to do now and I guess I just need some hope that if this is HSV 1 my life won’t be changed my outbreaks. I would love it if it wasn’t HSV at all, but I feel like it is given the painful urination, exposure (he didn’t have a visible lesion when this happened), and the one fluid filled vesicle. Please help.

      I am also suspicious that you may have acquired HSV 1 genitally. About half the US population has HSV 1 infection so it is very common.

      If you were my patient, I would recommend that you do another IgG test about 8-10 weeks out of the encounter in February. If your HSV 1 IgG test continues to be negative, you can say with 70% certainty that you don’t have HSV 1. That statistic is compared to the gold standard western blot. If you want to know with greater certainty if you have HSV 1, you could do the western blot 12 weeks out. And if you have any other blisters in the genital area, have them swabbed right away

      Would you recommend suppressive medicine? I’m new to this so unsure the likelihood of recurrence and transmission.

      If you have genital HSV one, you will likely shed very infrequently. We honestly don’t know the benefit of taking suppressive antiviral medicine for people who have genital HSV one because it’s so rarely is shed. If you have a partner concerned about your HSV one infection, you could consider suppression, although I honestly don’t know how much good it will do to reduce transmission since transmission of genital HSV one is so rare.
