      Hi,

      I’m 19 and I’ve only ever been with two people. The first was a relationship that recently ended and that I was in for four years. We were each others first and only. After the breakup, I had sex with someone else who was much, much larger than I was used to and after the sex, I immediately felt a few small tears and tiny drops of blood. From the moment after for two days I was in pain, but I thought it was the tears. I went to get tested at my campus’s health center to be safe and she did a culture for herpes. The results came back that I was type 2 positive for herpes, had a yeast infection, and had small tears that were infected with strep. I was also given 1 gram Valaciclovir twice a day. Within two days, the symptoms were completely gone which was only four days after the sex had occurred. However, I went to my normal gynecologist a week after the sex had happened and got a blood test done. The blood test came back negative for HSV 2. I have heard that an initial outbreak should be much more painful and last several weeks. Is it possible that the culture was a false positive because my symptoms went away within 2 days of them occurring? I am also very positive I was not infected with HSV prior to this as I have been tested before.

      I have an extra question for you assuming I am positive for HSV 2. My ex and I are considering getting back together. However, he wants to know what the chances are of him getting the virus as well. We would use condoms every single time and I am also taking 500mg Valaciclovir once a day. Could you give me a ballpark estimate of how likely it is that he will contract it? Also, does it make a difference in how long we wait to have sex after I have begun taking the Valaciclovir? From 2 days after my initial outbreak I took 1gm twice a day for about 10 days (symptoms cleared after two days) and yesterday switched to 500mg once a day.

      Has your other partner been tested to see if he is positive for HSV 2? If not, he definitely should be to help see how accurate that swab test was. Did you use condoms when you had sex with him? Did you talk at all about STI testing with him, like had he been tested? If I were you, I would absolutely insist upon him being tested for HSV 2.

      This is a bit of a double bind because if you continue daily antiviral therapy, the follow up antibody testing that I would recommend will possibly be inaccurate. But if you don’t take antiviral therapy, you are more likely to transmit to your old boyfriend.

      Can you let me know about the other partner being tested so I can help you sort this out?

        Yes, we did talk about STDs beforehand and he assured me multiple times he was clean. However I am obviously doubting that now. We did use a condom except he took it off for maybe the last minute or so and he did not finish inside of me. I have not talked to him since I got tested as he is a famous athlete at my university and I feel uncomfortable talking to him again after he most likely has lied to me and for fear he will deny it and tell people I have HSV and gave it to him instead. My main worry at this point is transmitting it to my ex boyfriend if we get back together. Again I would use a condom and am taking the 500mg of valaciclovir daily. Also if it makes a difference, the test done was an HSV culture with typing and it came back as type 2 isolated. What does that mean? Does it mean anything that bloodwork done a week later came back negative for all HSV? And how likely do you believe it would be to transmit the HSV to someone else?

      If the swab test is positive and the blood test is negative, that means this is most certainly a new infection – you have virus present on your skin but not enough time has passed for antibody to have been made. You have new herpes so you will be shedding more often in the first several months of being infected. I would say that if you use condoms every time you have sex and take daily medicine (I might suggest 1000 mg a day), then there is probably around a 3-5% chance you will infect your past boyfriend.

      And might I also add that just because he is a famous athlete, he most likely gave you herpes and you can prove it with the positive swab test and negative antibody test so no once can say it was you that gave it to him. I think you should consider making an issue of this!

      Thank you very much! My last question is if there is a certain time period you think I should wait until I have sex again with condoms and valaciclovir? I don’t want to move forward too soon but my ex is on board and wants to rekindle our relationship. And yes, I do plan on confronting him one day as we were friends initially but before I do that I want to become comfortable with the idea of having an STD.

      No, I don’t think there is a certain time to wait. Certainly you don’t want to have sex when there is a visible outbreak present. And you know there is some risk of transmission to your partner. And I do think either 500 mg twice a day or 1 gram once a day for the first several months might not be a bad idea as you shed more right in the early days of infection.
