I have a partner who is taking 500 mg of Valtrex once a day for about a year. In the past month, he’s has had one cold sore outbreak. He potentially had another outbreak a few months previous to this. Does this mean that the medication is no longer effective at preventing outbreaks/reducing viral shedding?

Nope. That is a pretty normal breakthrough rate in a year.
