Good afternoon and Thx for the chance of sharing our problems with you and asking for advice.
I had an exposure in which there WAS NOT sexual intercourse, a month ago , and I am just extremely worried mainly because two things :
During this encounter, I fingered the lady (by inserting one fourth of my finger) for 5-6 seconds, just enough to feel her moist. Then I pulled finger out, procced to wipe with linen, and then grabbed my penis and started masturbating while my scrotum was touching her left butt cheek. So with this being said :
1. Do you think the tip of my finger might have transferred herpes microbes from her fluid to my penis when I started masturbating? Or does genitalia skin contact between two persons +friction during a lapse of time must be present to consider transmition ?
2. Placing my scrotum area on her butt cheek worries me. While I was masturbating, I would just lay my scrotum there for seconds and then moving away from her. I repeated that approach-and-move away thing just two times I guess. There’s was not sign of lessions there and skin looked intact. Not I see any sores in vaginal area.
If she was infected, is there a possibility that she might have shed from her thick buttocks skin and that it could have been transmitted through my scrotum skin? That’s my main concern. I am sure I didn’t see any lession on the left cheek, but I couldn’t see if she had any on the right one.
Since exposure a month ago, I have been having ocassional back pains and ocasional pain and burning in anus now with no sores or blisters .. I recently saw a PURPLISH little thing there that looks like swollen vein INSIDE anal canal + pain and pressure though … Went to doctor and said it was hemorrhoids.
Would herpes ever look like a swollen vein or a purplish bump staying there for almost a week, without breaking open and causing raw painful sores like herpes blisters normally do?
And the lower back pain won’t go away…. having recurrent episodes of back pain during a whole month… Would that have anything to do with the presence of herpes?
I would to hear what you think about my situation..

Hello,
So there is absolutely nothing in your description of your sexual activities with this person that concerns me at all. Herpes looks nothing like a hemorrhoid so I think you’re safe there.The buttocks skin does NOT shed. While genital skin is thin and virus can be shed easily, the buttocks skin has a thick layer and thus shedding cannot occur when there is no outbreak present. I think your scrotum touching her buttocks is completely risk free.
Also, putting your finger in her vagina very briefly, then wiping it on a linen before beginning to masturbate does NOT present a risk to you. One of the things about transmission is that there has to be sufficient volume of virus present for infection to occur. I definitely don’t think this is the case for your contact. And I don’t think the back pain has anything to do with your contact, unless you moved your body in a way you normally don’t move it and it just kind kind of out of wack with a strained muscle.
It sounds like you were very carefully to practice safer sex in this situation.

Thank you so much for your reply Ms. Terry. I think Ignorance can make us go crazy sometimes… I have been extremely worried since that moment of exposure.
I just wanted to ask you A few more questions if you don’t mind… I am just curious and trying to understand :
Many websites list lower back pain as one of herpes symptoms…Is this pain caused because the virus travels back and forth through sacral nerves damaging the nerve as a result? And if that’s so, would it be a normal behavior for herpes to be causing Lower back pain ( virus traveling through nerves, or simply staying in dormant mode there) for a whole month without showing any lesions?
I know you mentioned my back pains wouldn’t have anything to do with my previous contact… Just have been having this lower back pain, and lightheadedness and now even spreading to knees. Would this have have been sign of herpes if I had had it for a month without any lesion?
Thank you some much once again… And apologize if I am being ignorant in some of my questions… It think there is no dumb questions though, Just trying to understand here.

Back pain, by itself with no lesions, is not a symptom of herpes, no. I don’t know what websites you are referring to but I doubt that any would say that by itself is characteristic of genital herpes. Nor is lightheadedness .
Your questions are fine – that’s why I am here. But we must go back to the sexual experience you are worried about. The things you did just didn’t put you at risk. Your scrotum touching her buttocks, with no lesions present on her buttock especially, is not a risks. Even if she did have lesions on her buttocks, I wouldn’t say that is much of a risk at all!

Ms Terry Thx So much for your reply. Just a few more things, if you don’t mind helping me again :
Couple of days ago I started feeling a slight burn like sensation (like if I was placing an ‘Icy Hot’ patch on that area) on my inner thigh area next to my knee on my left leg. It lasted exactly a day and then the sensation disappeared. While it lasted it was sensitive to touch and temperature. Coincidentially, I had been having this back pain I mentioned to you before for Almost 2 months (getting an MRI next week) but this ‘burning’ on my thigh shows itself 2 months later ( I hope is related to a damaged nerve or something).
I have understood that there is a ‘prodomal stage’ before an outbreak which lasts 1 to 2 days, in which a person feels a tingling sensation.
1. If there was a virus present, would you consider it Normal for the prodomal stage to stop without ending up on outbreaks, then ‘taking a break’ ( a day/weeks) and then come back later stronger perhaps, to finally get the virus out to the surface? Or even if this second attempt failed, to try a third time and so on? Does that Ever happen?
Also Ms Terry..
2. How accurate would you consider Test results 2 months after exposure?
3. What do those numbers in the test results mean? Are those amounts that represent quantity of antibodies? If someone is infected let’s say, and that person Takes a test 1month after exposure and it turns out to be negative showing X number of antibodies and 1 month later takes a second test and this shows a greater number of antibodies but still negative… Does that mean that the number of antibodies will keep increasing showing eventually a positive result if Takes a third test
I am planning to get tested but I need to wait one more month to complete the 3 months waiting frame. I know you told me that the type of contact I had with that person didn’t present much risk and sincerely appreciate your insights, but I guess the guilt is killing me and I can’t avoid relating symptoms ( like the possible back pain and nerve) with that encounter .
I would deeply appreciate if you can briefly answer this 3 questions Ms Terry … Thank you so much

1. Prodrome doesn’t last a long time like that. Normally, it appears for a few days before an outbreak.
2. The test would be accurate with about 75% certainty.
3. It is normal for the index value (the number you get with testing) to vary some from test to test. Each test is compared to a control sample and that varies from test to test. A decrease or increase of just a few points doesn’t mean anything.I think you are noticing normal body sensations more, due to your encounter. I could be wrong, of course, but none of these things sound like herpes to me.
If you’ve never had a previous test and you turn out positive at 12 weeks, then you cannot know if you’ve been infected for a long time or it is a new infection.

Good afternoon Ms Terry.. I am writing you again after almost two weeks… Kind of worried here..
A week ago I started feeling a sensation on the left bottom side of my scrotum. It is located on a very specific point.. So specific that there is a hair there that when I touch it it kinda of feels like a needle there ( I have to say that I shave my scrotum). It kinda of stings ocassionally when I don’t touch it ( that is when I am walking or just sitting) I wouldn’t say it tingles in that specific point ( follicule?) and a little of its surroundings in which I feel a sensation of irritation as well . It gets very sensitive when I rub it with tip of my finger. I went to doctor and got tested for HSV 2 ( after 2 months from contact) and did swab test on scrotum because he noticed it was red and thought there could be a bacteria there. He prescribed me an antbacterial cream. Sometimes the cream relieves it a little and shower but later I still feel it.
1) If this was herpes, Would this specific point that I touch with finger get all sensitive and even pinch if I touch it? ( I have to say that as soon I feel that sensation, it disappears as soon as I move… Gets interrupted)
2) It has already been a week Since sensation started. Would prodome last more than 4 days?
I also have to say I have been extremely worried, and been checking myself out every 5 minutes… Perhaps I have added bacteria there by doing that… Plus the fact that I shaved with I used razor couple of weeks ago…
Every morning I wake up, my only hopes are sustained by the fact that you said ‘Thick skin of buttocks and thighs don’t shed the virus’ … Ms Terry… Nerves do not go through thick skin then?
I would appreciate once again your insights …

My advice stands. Your experience was an extremely low risk for herpes and I think you are being very aware of your body sensations. I think you should try to stop checking your genital area and for now, stop shaving your scrotum. Just wash with antibacterial soap, dry well, consider using a little antifungal powder on the scrotum and leave it alone for half a day. You can look at lunchtime and bedtime, but other than that, I would leave it alone. The sensations of the scrotum that you describe do not worry me at all about you having herpes.

Just if you don’t mind answering this couple of questions…
Based on what you have seeing on other cases during your years on the field:
-What’s the longest that prodome stage goes before OBs show?
-Is it Always followed immediately by OBs? Or have you ever seen any case where it stops without showing lessions?
-What percentage of your cases would you say have shown symptoms during the first month after exposure?
If you don’t MÍnd just answering this briefly… Just would like to know a little of what you have seen on other people… The average. Figures also other people might find this informative.

No, we don’t usually see prodrome with first outbreaks.
