Hi Terry!
I had been having sex with a man from Nov to March. One time I saw a cut on his penis and he said it was from something like masturbating without proper lubrication. That was Feb 13. The last time he and I had sex was on March 19.
I had all of my STI/STDs tested On June 26. All my tests including the HSV igg 1 and 2 were negative. (18 weeks from the cut and 14 from the last encounter)
In the meantime, I had a small pimple on my inner labia. The DR said it looked like an abrasion from being sweaty, tight clothes, etc. He even made a comment about the color which eludes me now. He told me to use antibiotic ointment and within a few days to a week it was good as new.
I have a new partner. The first time we have sex is on Oct 24. On Oct 28, I feel sore on the right side up near the entrance of my vagina. It doesn’t get horrible but I feel it getting worse. The next time we have sex is the following Saturday. We had a lot of sex, however, it was naturally well lubricated. At some point that day, I told him it hurt badly and asked him to have a look. He said it looked red. By Monday, I felt a bump. I went to the dr. He didn’t think it was more than rawness from a lot of sex. He took a swab. By Wednesday I was feeling 2 bumps and the pain had increased. After seeing me, he said there were a few bumps; it looked like herpes, and did a swab. He prescribed suppressant medication and some cream. I started to feel feverish, etc. By Friday, the first test came back positive for simplex 2.
My partner says he will be tested. He says he has never had an outbreak. Don’t know him well,but I know that can be true.
My question is, do you think that the initial blood work, having waited 14 weeks since the last interaction with partner one were accurate? My dr seems to believe that they were conclusive and that my current outbreak is from new exposure. I have terrible guilt over exposing someone when I thought I was clear.

I would agree with your doctor that you infection is very likely new not old. I do think statistically, the IgG test 14 weeks out would likely have picked up the infection. There are exceptions – you could have been one. When the swab testing was done recently, was antibody testing done as well? That would have told you what you want to know. If you had a new lesion AND a positive antibody test, that would have told you it was old infection, not new.

I did not have an antibody test with the last cultures. Should I have one? I have the blood work rx. The DR wanted me to wait. Is it true that the blood test taken after a 2 and a half week exposure would not show antibodies yet? Even when having such severe symptoms? I would think antibodies would be present, but then, you are the expert! Don’t some people show antibodies as soon as 2 weeks or not?

Well, the thing is, if your blood test was positive with the first outbreak, you would have old infection. If you have a blood test now and it is positive, you can’t know if it was old or new infection but if it is still negative, then yes you would know it is new infection. Rarely do I see any kind of substantial index value on a blood test at 2 weeks out, no.

It is November 13. I had another blood test on the 11th. It was negative. Clearly a new exposure, yes?

Yes, in my opinion it is a new infection and you captured that via antibody testing. Now to prove it, you will need a test down the road (if you want or need to prove it) to document that you did seroconvert (that is, go from negative to positive). Usually there is no need to prove it since you have a positive swab test but just sayin’ because people ask me that all the time.

I think that getting a test in the future is a good idea in theory and as a safety net, although I feel that I now am certain that it is a new infection. That was my top priority and reason to worry in that I did not want to have been the one who spread it, unknowingly, to my new partner. There is no need to place blame at this point but it puts my mind at ease. Now I can begin to process the fact that I have HSV 2 and learn to live with this diagnosis, taking the appropriate precautions using food, meds, exercise, and supplements. I have some emotional work to do and acceptance and self forgiveness. Thanks you so very much for your input. I appreciate it greatly.
