Hi,
Since , more than a month i had some form of irritating pain around my genital area (groin) and suddenly on march 18, i noticed an ulcer on my penile shaft, it had a pinkish halo and a whitish gray base with no pain or itching. It was dry .I got an STD panel test the next day and all came back as negative. I went to a dermatologist after 3 days and got a biopsy done of the ulcer and it showed similarities of an apthous ulcer with no herpetic, syphillis or any STD bacteria or viral features in it. I was treated by a Infectious desease doctor for syphillis and chancroid and am currently on doxy 100 mg since 2 weeks and on valacyclovir for 2 days. The ulcer hasnt changed much since march 18th, but is now gradually healing. I had another test for herpes done 2 days agao i.e. IGG 4th gen and it came back negative . I got a PCR test done it came back negative. The last time i had sex, was 40 days ago, with protection.+
Can anyone shed some light on what it actually is?

First I would like to ask you where you live do you live in the United States? Has anyone discussed the possibility of LGV? Were you treated with injections for possible syphilis? Are you only being treated with doxycycline? Have you had a syphilis blood test? This is a significant ulcer and really doesn’t look a lot like herpes to me. You’ve had a negative IgG test and it sounds like you haven’t had sex for over a month so I would expect that if this was herpes you might be starting to develop some antibody. Were you tested for both HSV one and two IgG? I’m thinking that this is probably really bothersome to you and I wish I had some answers but it sounds like both the dermatologist and the infectious disease doctor are all over all the possibilities but I would be thinking of you as well. How long has the ulcer actually been there all together now?

I currently live in atlanta, GA. I did get treated for Syphillis and test after 1 week this ulcer appeared, it was negative in every test. My IGG test 3 days ago was also negative and right now, the Infectious desease doctor has put me on docy for last 2 weeks and since 3 days on valacyclovir . I thinki was treated for chancroid, chancre, LGV ( doxy 100mg for 21days cover it i suppose). I really dont know what it is, I hope it is not herpes which has no cure currently. It is getting better for the past 5 days. I am waiting on my herpes PCR results BTW.
The ulcer has been there since march 18th.

Good you have been treated for all of that. This just isn’t exactly looking like herpes to me but it is certainly a possibility but a negative PCR AND a negative IgG AND a negative biopsy suggest that’s not it as well.

hi terri,
Thank you for taking your time to answer my questions. Did you ever come across a HSV1 or HSV2 ulcer that looks like that or is it just as the dermatologist said after the biopsy that there seems to be no herpic viral features or bacteria seen in the ulcer . How long does it take for genital HSV to heal? its been almost 16 days since my ulcer showed up.

I would think if it was herpes you would be starting to see some healing by now. Have you been out the country having sex with anyone? Are there are any more lesions on the penis?

NO. the last time i had sex was 40 days ago, with protection till the end. I had some pain in that area on my penis for couple of weeks, didnt see any sore or anything there though then. I masturbate a lot daily, but one day, there suddenly appeared an ulcer on that spot . I had some tingling pain on and off near my groin for more than a month prior to having that ulcer.
No other ulcer near or on my genitals. The biopsy was done 4 days after the ulcer appeared.

Goodness gracious. The pain prior to the ulcer is suggested a genital herpes but if the biopsy didn’t pick it up that seems far less likely. I’m a little confused. In your initial post you mentioned that the PCR was negative but in your most recent post you said you are still waiting for results. Can you clarify? So my question was have you been out of the country and had sex with anybody outside of the 40 days? Like before the 40th day? There are a couple of specialists trying to evaluate the information that you have given me and the picture to see if they have any other ideas about what might be going on. I’m hoping to hear something back from them and when I do I will certainly pass it along to you. See what you started? LOL

Sorry, the PCR iam still waiting. I never had sex outside the country . The las time i had sex was 40 dasy ago in the country. I was administered a pennicillin shot initially for syphillis, that cured almost most of the pain in my groin and doxy cleared me the rest. But still dont know what is wrong, since the ulcer still there.
