I am a 40 yo male. Had a casual protected vaginal sex year and half ago.No oral sex. One night stand. Partner status unknown. Worried with guilty feeling and fear of contracting STDs from next day onwards.
A week after the encounter, I Started feeling itchy and irritation at the foreskin of penis. No sores. Started googling for genital herpes symptoms and kept on eye on genitalia for any sores.
Experienced fever, headache for one day after three weeks from the encounter. Started experiencing itching, pain in butt. Also experienced pain in left leg with pulling sensation for few days.
Right after a month from the encounter, Woke up with sweaty and itchy groin. No liquid filled blisters.
Then there was one pimple like bump on my right bum. Itchy and painful, but hard to touch. After one month another pimple like bump with similar appearance occurred on my bum and went away in three to four days. From that day onwards, I started experiencing constant itch on testicles and base of the penis but never any sores or blisters.
In and off I see pimple like red hard bumps on my thighs (always single )
However, recently started seeing pimple like red bumps below my belly button(always single bump) hard to touch. But painful when touched. They contain pus and are painful. But then I pop them and take the lid out pain subsides and eventually it leaves a scar.
Attached are the pictures. But I am still experiencing itchy anus, testicles even after year and half. I never had vaginal sex after that incident with the fear of spreading the disease to my partner. However I fingered her few times with my clothes always on . Never let her touch my genitals or boxer short area. But these days she is also complaining of Anal itching, but no other symptoms.
I am really worried and stressed. Do these symptoms suggest herpes?

Your symptoms do not suggest herpes at all. You’ve been worrying about this for 18 months and haven’t done any testing, is that correct? I am assuming that you have a regular partner and this was someone different? A single protected sexual encounter is extremely unlikely to result in herpes. I think your guilt and fear about this incident have you overly observing symptoms in the genital area.

Yes, I have always been monogamous until the casual one night stand with a stranger. However from that onwards all the bumps/blisters I experienced left scars. I never had any skin problems throughout my life.
My spouse complaining about anal itching and a single bumps last two weeks (one on her thigh and another in her bum) both one week apart is making me nervous and guilty if I had passed anything to her.
My symptoms of pain and itching testicles and groin (sometimes) is making think I have some issues which is now supported by my wife’s conditions
Do you still think I have some issues? If yes do they still suggest herpes? Please advise.

The only way I agree to view pictures if you become an online patient of mine. To do otherwise is just not legally OK.
Have you considered doing an IgG test for herpes antibody?

I think the chances of you having genital herpes based on the symptoms you’ve described are really low. I cannot say zero but very low. If you do the test and want help with interpretation of the results, let me know

I say not zero because condoms do not give 100% protection when it is an uninfected male having sex with an infected female. But a single encounter, with no lesions, makes the risk very close to zero.

Apologize for the delay in getting back to you.
I recently took a 10 panel test along with Trichomoniasis test. Everything came back normal.
Now question: what are the chances of IGG shows a false negative as I never showed any classics herpes symptoms? (I haven’t had sex with anyone in the last more than one year)
I am still getting those hard to touch single bumps even today occasionally on my things or pubic area (never on penis or scrotum)
But I have constant itching of scrotum, base of the penis and anal area.
Do you still think my herpes result could be false negative? Should I consider another test? If herpes is ruled out what less this could be?

The IgG test misses 8% of HSV 2 and 30% of HSV 1, when compared to the gold standard herpes western blot done in the same person.
