Hi Terri, I am really happy that a platform like this is available to get your advice. I am 33 Y uncircumcised unmarried Male.
Exactly last Friday morning, noticed a red rash and slight discomfort while urination with visible red dots on only the right side of the shaft. Used Tylenol for relief but out of anxiety, rushed to Zoomcare (urgent care) by 4 PM the same day. By evening at Zoomcare, there is not much redness, and the clinic diagnosed the absence of any nitrites or Leuks in my urine sample. Clinic found 2 red papules, with no scaling, no vesicles, no pustules, no pain, no oozing or warmth nor any penile discharge and diagnosed my condition as Balanitis, and gave me an anti-fungal cream. Applied it overnight and there are no signs of any aberrations the next morning. But I am still using the cream till today.
I had an unprotected encounter couple of times in the last 3 months, but I wanted to clarify my doubt by giving STD panel test @ Labcorp. All other STD tests came normal, except HSV 2.
03/31/2023 – HSV-2 Type Spec Ab, IgG w/Rflx – Index value of 1.20 marked as Low positive/ abnormal
Retested for supplemental test @ Quest for HSV 2 on 04/04/2023, Index value of 1.29 marked as Abnormal
Now I am waiting for HSV 2 IGG INHIBITION, IA confirmation test from Quest.
Usually, I get STD tested every 6 months. So my routine test on 01/05/2023, Herpes II came negative, but 03/31/2023 came abnormal/ low positive.My present situation: Waiting for HSV 2 Inhibition result, with no visible signs of pain or discoloration. But I’m depressed,anxious, stressed with this waiting how the results may come out. Given my situation,test results, & symptoms,please advise if there is anything worrisome or to be concerned about. Also, if the inhibition assay comes negative, does it mean I will not have HSV2? Does low positive mean I may not have HSV2?
I will be getting married late this year and I would be devastated if the results would come otherwise. Please advise Terri

So you do have a low positive for certain. The LabCorp test – no supplemental test was done? They seem to be out test kits right now. If the inhibition assay is negative, that is good news. If positive, I wouldn’t believe it as we have seen several false positives on that test as compared to the gold standard western blot. If you test positive on the IA, get a western blot to be certain.
You mentioned that you had had unprotected encounters in the past three months – was that with your fiance or someone else? If someone else, did they get tested?

I ordered my tests on Healthlabs.com to get tested with Labcorp. When labcorp results came abnormal with 1.20 index, Healthlabs told Labcorp couldn’t perform supplemental and ordered Supplemental and Inhibition through Quest for me.
Quest gave 1.29 index on IgG and right now I’m waiting for IA results.
Can having balanitis show low positive / abnormal results as I gave blood on the same day when I was with balanitis? Also right now, I’m not having any symptoms or abnormalities.
Regardless of both the lab results, do you advise me to ignore them and just rely on the western blot results?
This morning, I’ve scheduled a consultation with you on Apr 11th and shared these lab reports. I’m willing to take western blot through you after the consultation.
The encounters were with a lady I met through a dating site and I’m not sure if she’s infected or tested before because we are not in contact anymore.

Hi Terri, just got the results – the HSV2 Screening and % Inhibition from Quest came negative / normal. Quest is asking to resubmit blood sample in 2-3 weeks for another confirmation test. Should I take it or take western blot and then decide?
But what do you infer and advise to me in this situation?

The only reason to take an additional test is if you have had a recent sexual encounter and tested too soon for antibodies to have a chance to be developed.
So I don’t fully know the timing of the other sexual encounter but we can talk about that more when we meet on Tuesday to try to nail down the appropriate date for Western blot testing. I think it’s very good news that you’ve got a negative inhibition assay. We have seen positive ones that are false compared to Western blotTerri
