47 year old M great health no priors. Timeline:
Fri Oct 9 unprot oral vaginal sex as giver with 54 yr old F. Had fishy odor noticed after. Didnt see sores. Mon masturbated. Felt prosate tingle. Tues – Fri dull ache at penis base underneath behind scrotum and anus. No pain urinating. Sat it subsided. Mon 19 took lysein arginine. Tues feelings back. Got full panel (hsv1,hsv2,clam,gon,syph,heps) – 11 days after. All -ve.
Wed seemed to go away. Fri buzzing in top left leg like phone getting text no other symptoms.
Sat (Oct 24) no sores anywhere. Masturbated twice. Dull ache / tingle back again, pain scale of 10 (2). Penis head little tingly. Went to Emerg. Dr – might be herpes but maybe prostate infection. Gave Valacyclovir 1kmg 7 days – started Sat. Sun same symptoms.
Mon little more. Thighs both little weak. Night went for walk in nylon pants with mesh. Noticed hair folicle on left testicle to penis base had bled slighty – maybe sore but didnt look like it no puss. No penis sores, foreskin little tender. No real pain urinating. Yesterday mostly subsided 3pm. Went for walk. Slept good. Today noon came back. Legs weakish dull ache in place I imagine prostate is. Scrotum not sore/tender. Feel it when sitting. Dr said could be prostate infection but Valacyclovir first and back if not fixed. Have felt dull ache in past prior with cialis took 100mg sildenafil contact night.
After 19 days I have no sores. follical sore gone. Small sore above wrist but could be a nick 2 days ago. Dull ache and little tingling. My legs / thighs are little tingly and shaky. No itching, anus not sore either. Few pimples on butt not abnormal. Daily temp 97.8 constant. Not tired / run down.
Does this sound like an initial herpes infection after 19 days ? – is this typical to come n go ?
Whats my next steps ? – stay on the meds after ?
Could this be a prostate infection instead from fishy type odor or something ?
Back at ugent care in a day

It does not sound like herpes to me, no. Not at all. But it does sound like you may have some prostate issues going on here. You are about the right age for this kind of thing to start. I am also just slightly concerned about the fish odor. Can you clarify the kind of sexual contact again? Oral vaginal as giver? is that intercourse and oral sex? If intercourse also, I wonder about the possibility of trichomoniasis.
If you continue to stay on herpes medicines, you are interfering with the results of the herpes antibody test. If you want an accurate test in the future, I would recommend reconsidering taking these medicines. A final screening test at 12 weeks should be adequate to roll out herpes.Terri

Terri, thank you for the response. I went back to UrgentCare this morning after a night of a lot of pain in the prostate vicinity. After a very unpleasant procedure he agrees with you and put me on Cipro for 10 days. I also talked to my friend about this and she said she is checked regularly but didn’t know if that included all std’s but she had never had any symptoms other than a cold sore on the lip a few years ago but has regular paps and blood work etc. not with other partners either. It was intercourse also. if it was trichomoniasis i assume the Cipro would also take take of it ? or not ?
I’m going o stop the other meds and see if the Cipro clears this all up based on yours and the dr. input. Then retest at 12 weeks.
Thank you so much for the response. The internet is a great thing but it can also freak you out to much at certain times.

I completely agree that the internet is a mixed bag. It allows me to give information to others but those with less knowledge about herpes also can get their two cents in and that is not always accurate or useful. Guess we have to take the good with the bad and hope people can sort out the difference – unfortunately,that is not always the case.
A visit to urgent care is a great start and it sounds like a course of cipro may be just the ticket, but I would also recommend a trip to a urologist for follow up. Sometimes a prostate infection can take several weeks on antibiotics, not just 10 days, as the prostate is difficult tissue for antibiotics to penetrate.
Cipro is not a treatment for trich. That requires a different antibiotic. But honestly, I seriously doubt that even if you had trich that it would cause all these problems – it could be a second issue if any issue.
