I would agree that what you describe around the head of the penis sounds much more likely to be pearly papules than anything else. They definitely do not sound like herpes. No question about that. Your high HSV-1 index value indicates that you are most certainly infected with HSV one somewhere. Without a lesion in either the oral or genital area it is not possible to know which one is infected.
