Medical Records Requests
Would you like a copy of your medical records? We can provide those to you in the form of a PDF sent directly to your email from our secure email program. Just send a request to requesting your records. We normally send two years of records – if you need more, please let us know.
Do you want your new health care provider to have your records? We will no longer be sending your medical records to other clinician offices. Now that you have your own records, you can decide how your provider will get your records – you can send them to their office as a PDF or print them out and take it to their office. You are in total control of what happens with your records.
However, there are two exceptions. If you are a woman following Sheryl Horwitz, NP, to Synergy Women’s Health, we are able to link our electronic records to them directly. If you are going to Synergy Women’s Health Care, let us know and we’ll get your records to them directly.
If you are a male following Sheryl Horwitz to NW Dermatology, we can provide a direct transfer of electronic records to them as well. These are the only two practices to whom we will be sending records directly.

I want to order Updated Herpes Handbooks – how do I do it?
We still are filling orders for Updated Herpes Handbooks for individuals and institutions. Please send your order to and we will be pleased to fill these orders quickly. If you have questions about prices or shipping, please include these in your message.
Can I stay with my same clinician somewhere else?
You have two options:
Nurse practitioner Terri Warren will be available for herpes consultation via video (see that box on the home page) after June 1st and via the forum (another box on the home page). She will also continue to enroll patients in the herpes western blot study via phone call (503-226-6678) until June 1st and after that, via video conferencing. She will no longer be seeing patients in person.
Nurse practitioner Sheryl Horwitz is seeing patients in two locations:
Women can see Sheryl at Synergy Women’s Health Care at 2250 NW Flanders, Suite 205, just a block from our previous clinic location. Their phone number for appointments is 503-227-4050.
Men can see Sheryl at NW Dermatology & Research Center. They are located in our previous building at 2330 NW Flanders, Suite 201. She is there on Fridays. Their phone number for appointments is 503-223-1933.
